RN-FT Days | Iron Bridge Recovery Center | Chester, Virginia About the Job: PURPOSE STATEMENT: The Registered Nurse is responsible for providing nursing care to clients. Supervising care treatment and services of client care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each client. Assessment and re-assessment of clients. Communicating appropriate information with the interdisciplinary team. Roles and Responsibilities: ESSENTIAL FUNCTIONS: Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. Assesses clients’ status on an ongoing basis and communicates finding to the providers timely. Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. Obtains provider’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews provider’s orders ensuring all required elements are present and appropriate per policy and procedures. Accurately administers medications, per ordered ,utilizing at least two client identifiers. Notifies providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or change in medical status regarding response to medications, etc. Assesses and documents effects of PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and escalates coverage needs to supervisor/Charge RN. Ensures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Communicates thorough hand-off reports to oncoming staff and supervisors. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. One or more years’ nursing experience within clinical area of service provided by facility preferred. Experience in behavioral health/psychiatric/addiction recovery environment preferred. Knowledge of and experience with psychotropic medications and medication administration procedures. Ability to apply nursing standards and practice within an acute psychiatric setting LICENSES/DESIGNATIONS/CERTIFICATIONS: Valid current RN license from the state or from a compact state, where applicable. Certified in Psychiatric Nursing preferred. CPR and de-escalation and restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility requirements. WORK LOCATION: This position is onsite at the facility and is not a remote position. Minimum of one (1) year experience in nursing, preferred. Acceptance of the philosophy and goals of this Agency. Ability to exercise initiative and independent judgment. Principal Responsibilities: Understands and adheres to established policies and procedures. Implements the nursing care plan for each patient. Provides nursing services, treatments, and diagnostic and preventive procedures as assigned. Initiates preventive and rehabilitative nursing procedures as appropriate. Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Evaluates with registered nurses the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse. Maintains accurate and complete records of observations, treatments, and care of patients. Participates in medical record audit as assigned. Attends staff meetings, patient care conferences, and in-services as scheduled. Takes on-call duty, nights, weekends, and holidays as assigned. Is responsible for: Submitting any changes in schedule to the Director of Clinical Services/Clinical Manager daily. Participating in patient care conferences to discuss the need for the involvement of other members of the health team, such as physical therapists or speech-language pathologists Prepares clinical and progress notes. Assists the physician and RN in performing specialized procedures. Prepares equipment and materials for treatments. Observe the aseptic technique as required. Assists the patient in learning appropriate self-care techniques. Preferred Qualifications Home care and home health background. Knowledgeable with healthcare regulations, HIPAA compliance, and patient privacy requirements. Knowledge of the local healthcare market and community outreach strategies. Knowledge of appropriate software including Microsoft Word, Email, Outlook, and Wellsky (EHR). Clean driver’s license with a clean driving record (Preferred).
