The Home Hospital RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model. The Home Hospital RN will access and document the care of patients and will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient’s home while maintaining high quality and safety standards. Essential Job functions: Performs and documents nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely or in-person at their defined home. Reviews and follows-up on patient education, implemented patient care plans, and coordinated services. Triages, identifies, and acts on changes in patient condition. Utilizes and instruct patient and family on the use of technology resources used for biometric data capture, patient symptom reporting, and telehealth visits. Provides direct bedside patient education and consultation on continuum integration services as needed.
