HOW YOU'LL WORK: You'll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change. Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS: Registered Nurse with current license in the state of employment. Minimum one-year experience as an RN in an acute care setting is preferred. Hospice experience is desired. ESSENTIAL FUNCTIONS Prepares treatment rooms for examination of patients. Escorts the patient to the examining room, obtains a brief history, documents chief complaint takes vital signs to include weight and temperature. Preps the patient for exam, assists the physician/clinician on exams and any special procedures. May travel to off site locations to perform diagnostic testing and assist in physical exam components, TB testing, Injections, phlebotomy and others. May administer and record injections and TB testing under the supervision and order of the physician. Performs venipuncture and obtains specimens, performs CLIA waived lab tests. Performs office procedures including, but not limited to, PFT, UDS, EBT, EKG, fit testing, within the scope of practice as directed by provider. Receives telephone calls from patients, pharmacy and others; documents and relays information to provider for action. Phones pharmacy with approved medication orders/refills. Performs a variety of front office duties as needed, including, but not limited to, reception, telephone, chart maintenance, insurance forms and scheduling. Organize and communicate with insurance companies for authorizations for therapies, testing and specialist referrals. Provides appropriate referrals to case management for employees. Serves as a resource nurse for staff members, orienting new staff and providing feedback on nursing skills for specialty care areas. Develops and maintains care standards knowledge bases for specialty care areas and develops care guidelines for unit-specific individual populations. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards.
