Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21. Under the supervision of the Administrator, the Director of Nursing has the authority, responsibility, and accountability for the functions, activities, and training of the nursing services staff, in addition to the supervision of the Therapy Department and its’ functions within the facility. Key Responsibilities: Provide individualized support to help individuals participate in community activities, recreation, volunteer opportunities, and social events Assist individuals in developing friendships and building natural supports Encourage independence and self-advocacy Support participation in religious, cultural, and community activities when desired by the individual Assisting with daily living skills, personal care, and health support Provide transportation to approved activities and appointments when required Complete accurate and timely service documentation Qualifications: High School Diploma or GED Valid driver's license and reliable transportation Ability to pass all required background checks and screenings Ability to complete required training and certification requirements Strong communication and interpersonal skills Compassionate, patient, and dependable Ability to work independently and exercise sound judgment Preferred Qualifications: Experience supporting individuals with intellectual and developmental disabilities OR Experience in healthcare, caregiving, education, behavioral health, or human services CPR, First Aid, or Medication Administration certifications Bilingual candidates are encouraged to apply Physical Requirements: Ability to travel throughout assigned service areas Ability to lift, transfer, or assist individuals when required by the support plan Ability to stand, walk, bend, reach, and perform physical activities associated with community-based support services Ability to respond appropriately in emergency situations Pay $18.50-$23/hour Benefits Weekly Payroll Health Insurance 24-hour Support from the Kindly in office/on call team W2 Employee Status What Makes Kindly Different: At Kindly, we recognize that exceptional care starts with exceptional caregivers.
