Provides and documents daily care of patient per unit standards. In monitored units, follows telemetry process and collaborates with central telemetry for initiating/discontinuing monitoring. Identifies and communicates new resident/patient care issues/concerns. Communicates changes in patients/residents condition. Takes rapid action in life threatening situations and immediately notifies the licensed nurse. Communicates or questions requests outside of scope of practice to appropriate personnel. Communicates plans to transfer patients / residents to other departments or discharge to home. Communicates incomplete work requiring follow-up before the end of shift to licensed nurse. Safety Complies with universal precautions and hospital infection control policies. Uses proper body mechanics and available support equipment to ensure patient’s safety and avoid personal injury. With collaboration of the attending physician, writes antibiotic and/or oxygen orders needed for post discharge. Performs peer to peer appeals on post acute denials. Acts as back up for the Physician Advisor handling peer to peer appeals and written appeals when necessary. Performs secondary clinical assessment and documentation to support inpatient admissions as well as to support subacute, long term care(trach weaning necessity/settings) post discharge. Assists denials staff with open communication and feedback to medical staff in regards to denials due to level of care and insufficient documentation. Supports care coordinators to provide real time feedback to providers for improved documentation that will defend the length of stay. Works collaboratively with care coordination manager in process improvement in regards to length of stay, denials and readmissions. Supports care coordination department with difficult discharges and/or refusal to leave facility to a more appropriate level of care. Qualifications Bachelor's degree in Nursing required. Current NJ APN license required. Previous healthcare experience required. Previous experience in an inpatient hospital setting preferred. Experience with EPIC preferred.
