Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. The RN Case Manager in Home Care and Hospice plays a critical role in coordinating and delivering high-quality, patient-centered care to individuals receiving services in their homes or in hospice settings. This position is responsible for managing a caseload of patients, developing individualized care plans, overseeing implementation of clinical services, and ensuring that care is provided in accordance with medical standards, agency policies, and regulatory requirements.
