Utilizes appropriate interpersonal communication which facilitates productive relationships with other team members. Facilitates a positive and caring relationship with others by utilizing trust and mutual respect. Establishes a therapeutic relationship between patient, family and self, based on empathy and trust. Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Clarifies any unclear communication between members of the healthcare team. Uses proper channels of authority in communicating information. Collects Data related to physical, psychological and psychosocial health status. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises same as diagnosis and needs dictate. As part of the nursing process, performs admission, handoff, transfer and discharge procedures. Coordinates with physician’s plan of care, the admission assessment and discharge needs. Maintains compliance in RQI. Assumes all other duties and responsibilities as necessary. Charge Nurse: In addition to above essential functions, the charge nurse will also : Complete daily nurse leader rounds. Perform shift checks as assigned. Assists with completion of chart audits for nursing documentation as required. Complete staff assignments taking into account the skill level and acuity of patient assignment. Manages throughput on the nursing unit and make changes to assignments as needed. Reports to the nursing unit manager any problems in regards to performance, safety, staffing, behavior, etc. Assists with conflict resolution in the absence of the nursing manager in collaboration with the house supervisor. Role models positive behaviors with staff including integrity, fairness and consistency. Agent for change for the department. Assigned and assist as super user for completion of incident report. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Be the Best Hospice Aide You Can Be If you meet these qualifications, we would love to meet you: Certified by the state as a nurse’s aide, with completion of required training and a passing score of 85% or higher on the agency’s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD). CPR certification required for direct patient care in Georgia and Washington. Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals. Requires a reliable vehicle, valid driver’s license, and state-required auto insurance. Responsibilities: Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance). Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care. Monitor comfort levels, respond to needs, and assist with daily living activities, always maintaining patient safety and dignity. Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols.
