Assist clients with following a written, special diet plan and reinforcement of diet maintenance, which is provided under the direction of a Physician and as identified on the care plan. Escort clients to medical facilities, errands, shopping and outings as specified in the care plan. Assist in basic client transfers providing the client has been assessed as being capable of ambulating without assistance; and/or, providing another trained caregiver (including family) is involved in the transfer. Assist clients with communication by writing or typing correspondence for them or researching information for them. Provide companionship, friendship and emotional support. Talk, listen, share experiences, play games/cards, read to client etc. Help keep clients in contact with family, friends and the outside world. Provide transportation to medical appointments, grocery store and errands. Accompany clients to recreational and/or social events. Assist with plans for visits and outings. Participate on the Care Team by providing input and making suggestions. Ensure service is delivered in accordance with all relevant policies, procedures and practices. Monitor supplies and resources. Evaluate the program and make recommendations to it, as indicated. Follow the written care plan. Carry out duties as assigned by the Supervisor. Overview The Admission RN initiates all aspects of the patient’s home health care by performing the start of care assessment visit. At the initial assessment, explains patient’s rights and expectations, HIPAA as it relates to confidentiality of medical information and OASIS data, advance directives, and informs the patient and family of expected disciplines and frequency. Reviews changes as they occur throughout the episode of care. Develops a plan of care that corresponds with physician orders, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family in the planning process. Makes revisions to the plan of care as patient status and needs change. Communicates as needed with and updates the primary physician when necessary and at least every sixty (60) days. Re-assesses patients within 24-48 hours following a change in condition or discharge from an inpatient setting and updates the plan of care. Uses health assessment data to determine nursing diagnosis. Establishes positive working relationships with physicians and physician’s office staff and keeps them informed of patient’s problems and progress towards goals as appropriate. Nursing ECMO Specialists work in the Cardiac Intensive Care Unit and communicate with physicians, perfusionists and other members of the healthcare team to interpret, adjust and complete treatment specific care for the ECMO patient. ECMO specialists are expected to perform required technical skills with efficiency, accuracy and safety while anticipating, troubleshooting, and managing crises and emergency situations according to policy and procedure. Performs scheduled, un-scheduled, and emergency ECLS in the hospital setting. Operates all ECMO equipment under the direction of MD/perfusion providers.
