Advanced Practice Provider, Nurse Practitioner, Physician Assistant - Cardiac Surgery OR - Full-Time, Rotating

Join UChicago Medicine , as a Physician Assistant within the Cardiac Surgery department. We offer advanced surgical solutions to treat atrial fibrillation, coronary artery disease, heart failure, valve disease and many other heart conditions. From reconstructing failing hearts to performing robotic and minimally invasive cardiac surgery, our surgeons are helping patients with heart disease return to active, full lives. At the core of our heart surgery program is a team of internationally known cardiac surgeons who are leaders in the field and are often among the first to perform groundbreaking procedures. The University of Chicago Cardiac Surgery department is a robust program for heart transplantation, mechanical circulatory support including extracorporeal membrane oxygenation (ECMO), durable Left Ventricular Assist Devices (LVAD), bloodless surgeries and other high-risk cases. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. As an Advanced Practice Provider , you will be responsible for comprehensive direct patient care management and OR surgical assist. In this role, the APP will manage a caseload of patients and exercise autonomous decision making in the assessment, diagnosis and initiation of delegated medical treatments. The scope of practice includes operating room support, consultation, patient management, discharge planning, staff education, consultation and research. The APP will utilize advance practice skills obtained through the completion of a formal provider program. These skills include advanced levels of assessment, planning, intervention, and evaluation of care. The APP is also responsible for planning, organizing, and directing patient care efforts in collaboration with medical and surgical physicians and other team members. Eligible for a Sign-On Bonus up to $10,000.00 Essential Job Functions Gathers pertinent information systematically and skillfully from all sources: history, diagnostic tests, comprehensive physical examination, and psycho-social assessments. Differentiates normal and abnormal variations for all body systems. First Assist to the cardiac surgeon with all procedures Endoscopic vein harvesting Assisting with percutaneous intra-aortic balloon placement Accurately and appropriately performs routine diagnostic and therapeutic techniques according to established protocols and standardized procedures for nurse practitioners. Assesses patient and family adaptation, coping skills and the need for crisis or other intervention. Accurately interprets diagnostic tests. Analyzes multiple sources of data in making clinical judgments and in determining the effectiveness of a plan of care. Develops problem lists with associated differential diagnosis. Performs bedside procedures as appropriate to the patient’s clinical needs Incorporates assessment findings into a plan to direct patient care. Formulates a plan of care in collaboration with the patient, family, physician, nursing staff and other member of the health care team, which incorporates health care maintenance, discharge, and follow-up care. Assists, on an as needed basis, in other areas such as inpatient care and consultation services. Plans and implements appropriate pharmacological therapies. Initiates referrals based on patient and family need. Seeks appropriate consultation dependent upon patient health care needs. Accurately presents and documents the database, impression and plan of care. Promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of adolescent, adult, and geriatric patients. Participates in research projects in the practice area. Attends and actively participates in resident care plan conference, and Resident Council and/or Family Council when invited. Reports all concerns, complaints made by the customer or their family to the Administrator. Makes rounds to assure customers are safe and comfortable. Maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Participates in In-service Education Programs. Assists in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department, and its policies and procedures. Recommends to the Director of Nursing the equipment and supply needs of the department. Inventories and stores the new customer’s personal possessions. Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. JOB SUMMARY The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility. SUPPORT DUTIES Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff. Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training. Encourage family involvement through volunteering and visitation. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach. Be flexible to meet the change in residents’ needs. Flex Emergency Services Admissions RN - 12-hour shifts (7am - 7pm)

Responsibilities POSITION SUMMARY: Focuses primarily on RN-to-RN screenings, (i.e. reviewing H&P, labs, and other clinical documentations) for all admissions and ensures patients are medically appropriate for admission. Responds to inquiry calls regarding services. Oversees the admission process including the appropriateness of the admission, completion RN to RN screenings, orders for the admissions, and any additional required paperwork. Assists in assigning unit/program based on patient needs. Knowledgeable and able to explain Hospital programs and services. Provides accurate and complete information to physicians regarding RN-to-RN screening. Communicates with physicians and providers to receive admission orders and medical clearance as needed. Assures compliance with all applicable laws, codes, rules and regulations that pertain to psychiatric admissions. Maintains data/records regarding the admission process for medical screenings. Completes medical screening evaluations and triaging on all patients who present for an assessment in accordance with EMTALA laws. Provides first aid and medical care as needed. Coordinate transfer of those patients requiring a higher level of medical care and/or medical clearance. Has a strong understanding of the department computer systems. Conducts the initial assessment which includes prior treatment history, previous medications, health screen, family dynamics and risk factors. FLEX/PRN position - no benefts.
