CentraState Medical Center currently has an employment opportunity available for a Clinical Leader (Assistant Nurse Manager) in the Medical/Surgical (ACE) 3-North unit. The Clinical Leader plays a pivotal role in coordinating culturally sensitive care from pre-admission through post-discharge. This includes managing resources to achieve optimal clinical and financial outcomes, supporting quality service delivery, and guiding staff in evidence-based nursing practice. The role also assists the Nurse Manager in overseeing unit operations on a 24-hour basis and serves as a clinical resource and educator to team members. Care is delivered through a multidisciplinary, patient-focused model aligned with CentraState’s organizational goals and standards. Responsibilities Assists the Department Head in the continuing effort made by the department to improve the quality of patient care and services. Provides considerate respectful care focused upon the patients' individual needs. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the patients cared for. Demonstrates an understanding of the range of care needed based on the age of the patients cared for. Successfully completes life-span education as appropriate. This assessment is communicated effectively and documented. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medications as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Attends and participates in the multi-disciplinary discharge planning meetings and identifies the need for further services following discharge. Provides appropriate education based upon teaching plan. Conducts unit based education programs on a routine basis in collaboration with professional development. Meets standards for documentation regarding patient education. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in Medical Records, Nursing Standards of Care/Practice for the specific patient type. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Delegates components of care to non-professional caregivers as appropriate and evalutates outcomes. Makes decisions that demonstrate knowledge of nursing practice. Utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the achievement of maximum outcome. Develops new clinical pathways, monitors patient progress on current clinical pathways, reviews variances with Patient Care Reviewers, reviews variances from Quality Management with the Director/Nurse Manager and staff and reports to Nursing Quality Council. Follows the clinical progress of all patients. Participates in the development of unit protocols as needed. Conducts unit conferences to discuss quality issues and quality improvement strategies. Quality Referrals - unit based, follows up on nursing referrals, investigates problem, develops action plan, reviews with Nurse Manager, responds to Quality Management in writing, implements action plan. Follows up on patient complaints or concerns and keeps Director/Nurse Manager informed. Provides or coordinates at a minimum, 1 formal in-service per year to address unit specific needs. Monitors appropriate usage of speciality and Zonaire beds. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy # 4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Description RN/Case Manager Mount Sinai West Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities Principle Duties and responsibilities include, but are not limited to: Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. Documents all clinical reviews in Canopy. Supports the mission, vision, philosophy and goals of the Medical Center. Case Management: The CM process will include: Assessment of the patient’s clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. Identification and documentation of variances affecting the LOS and the discharge planning process. Conducts follow-up of any delays in treatment or reporting of results. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. Documentation of outcomes achieved and identified internal and external barriers. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. Utilization Review: Maintains a working knowledge of the UR requirements of each payor within the patient population; Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. Works collaboratively with physicians and managed care companies on concurrent denial appeals Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. Liaison with financial department for current insurance coverage. Collaborates and participates in the appeals process with all members of the interdisciplinary team. Collaborative Relationships: Develops and maintains effective working relationships with interdisciplinary team and Case Managers within managed care organizations. Participates in nursing orientation. Interfaces with other departments in the Medical Center. Qualifications Bachelor’s Degree in Nursing; Masters preferred. Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years’ experience as a RN in an acute care setting. Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected.
