Job Summary: Kaiser Permanente (KP) nurses advance the art and science of nursing through clinical and service excellence, professionalism, and leadership. The Licensed Practical Nurse - Virtual Care provides virtual nursing care to patients, members, and families to promote health and wellness and prevent illness. Collaborates with clinicians, leaders, and members of the virtual care team to deliver high quality care in accordance with scope of practice. Assist with patient education. Documents clinical information in the medical record. Essential Responsibilities: Monitors electronic messaging related to patient and provider requests and inquires within scope of practice and as delegated by RN. Manages messages in a timely and effective manner to resolve patient requests and concerns. Determines appropriateness and acuity of needs based upon electronic messaging content and initiates follow-up telephone contact within scope of practice. Directs messages and transfers calls if outside of scope of practice to RN for management and resolution. Determines patient and family health education needs, provides health teaching and counseling and evaluates effectiveness of the education process. Accurately documents patient data according to established guidelines and policies. Participates in quality improvement activities or initiatives. Serves as an advocate for the patient. Completes other department-specific procedures and nursing care within scope of practice and skill. Adheres to KP compliance standards and Principles of Responsibility (Kaiser Permanentes Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licensure requirements (if applicable), and Kaiser Permanentes policies and procedures. Overview: Charge RN - Cardiac Care, Full Time Days *Sign on Bonus Available up to $5,000 Piedmont Eastside, Snellville, GA This 18-bed cardiac unit specializes in the care of both cardiac and vascular patients. The typical patient population found in the CCU includes but is not limited to patient's with diagnosis of chest pain, cardiac dysrhythmias, CHF exacerbation, hypertensive emergency, pulmonary embolism and/or DVT, and patient's that have had AAA repair and carotid endarterectomy. Our unit is an intermediate/step down unit. The nurse to patient ratio is typically 1:4 when caring for these patients. Our nursing staff work closely with both our cardiology and vascular teams to ensure exceptional care for our patients. Monitor seriously ill residents, as necessary. Review care plans daily to confirm that appropriate care is being rendered. Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident’s care plan prior to administering daily care to the resident. Other Duties Attend in-service education programs in order to meet facility educational requirements. Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information. Maintain confidentiality of resident and facility records/information. Protect residents from neglect, mistreatment, and abuse. Protect the personal property of the residents of the facility.
