CTRC Job Specific Competencies: The CTRC nurse has the competence and competencies regarding all aspects of administering and monitoring a variety of systemic cancer therapies and other injectable infusions to an adult population in the Clinical & Translation Research Center. PRINCIPAL DUTIES AND RESPONSIBILITIES: Clinic Operations: • Provides leadership, coaching, and development plans for all direct reports. • Partners with internal Human Resources, Quality, Education, and Technical Services departments. • Collaborates with the Home Therapies Program Manager to oversee the facility’s Home Therapies Program. • Maintains integrity of medical and operations records and complies with all data collections and auditing activities. • Accountable for completion of the Internal Classification of Disease (ICD) coding. • Responsible for all required network reporting and on-site state or federal surveys. • Manages the tactical execution of short- and long-term objectives through the coordination of activities with a direct responsibility for results, including costs, methods, and staffing. • Responsible for the administration of the daily business operations of the dialysis clinics including managing the functions and actions related to the center staff, quality of overall provision of patient care, maintenance of the physical plant and center equipment, and inventory control. • Manages the profit and loss and other related financial aspects for the center, ensuring optimal facility operations to achieve or exceed the budget and key performance indicators. Collaborates with staff and Charge RN and Medical Director to set annual goals for staff. • Manages the department staffing through the appropriate hiring, firing and disciplinary actions. • Maintains written documentation of all disciplinary meetings in accordance with the established personnel policies and confers with Human Resources and Director of Operations regarding the nature of the disciplinary decisions. • Ensures successful execution of new hire orientation and training, and ICD-9 code training when ICD-10 applicable for new hires and works with Medical Director to ensure mandatory in-services are completed. • Ensures appropriate documentation is completed for current licensure, annual in-service and policy and procedure in-service updates. • Responsible for ensuring all facility employees receive appropriate training according to company policy including company risk management initiatives. • Provides training and guidance to facility staff members to ensure development of clinical competences, providing opportunities for professional growth and encouraging personal growth. • Collaborates with HR regarding providing information to staff pertaining to FMS/FMCNA benefits, Human Resources policies and procedures. • Participates in Corporate and Division specific employee recognition and satisfaction programs. • Maintains a close working relationship with Division and Corporate office personnel and ensures appropriate communication of FMS, Division, and Corporate initiatives, policies and procedures to facility staff. • Utilizes knowledge of FMCNA and FMS services and products to contribute to the growth of the business. • Maintains facility environmental integrity and safety. • Schedules the maintenance and repair of equipment, operating systems, and physical structure of the facility, as needed. • Monitors the security of the facility. • Coordinates inventory/supply management by reviewing monthly inventory count and approving purchase orders for necessary items to ensure cost containment, timely distribution and competitive pricing. • Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the FMS/FMS formularies. • Contributes to the development and revision of any applicable policies and procedures for the facility and the pertinent updating and maintenance of the related manuals. • Directs any necessary information gathering, as required, to support billing, billing issues and collection activities. • Coordinates and approves facility payroll. • Responsible for participating in all on-site internal and external (state and federal) surveys. • Reviews new and existing provider contracts for various vendor services, including but not limited to, insurance, laboratory, and facility cleaning. • Performs other related duties as assigned. Job Description We are searching for a Registered Nurse for our 15WT Med/Surg Unit – someone who works well in a fast-paced setting. 15 WT is a 36-bed med/surge unit; which provides care for pediatric patients under PHM (Pediatric Hospital Medicine). Patients under the care of PHM are typically healthy patients that require hospitalization usually due to an acute event, i.e. status asthmaticus, cellulitis, bronchiolitis. Patient age ranges from a few days old to teenagers and young adults. Developmental ages vary by patient. Required Qualifications • At least 2 years of experience in health care, including at least 1 year experience in care management, managed care, and/or experience in a medical or behavioral health setting, and at least 1 year of experience working with persons with disabilities, chronic conditions, substance abuse disorders, and long-term services and supports (LTSS), or equivalent combination of relevant education and experience. • Registered Nurse (RN). Summary The Advanced Heart Failure Nurse Practitioner / Physician Assistant provides comprehensive clinical care across outpatient, transitional, and inpatient settings within the Advanced Heart Failure and Cardiac Transplantation Program at Texas Heart Institute / Baylor College of Medicine. The APP independently evaluates and manages patients with advanced heart failure, pulmonary hypertension, and complex cardiology conditions at the Baylor McNair Clinic and Baylor St. Luke's Medical Center Heart Failure Clinic, with periodic inpatient support for acutely decompensated patients. Responsibilities include medication management, hemodynamic optimization, diuresis protocols, transitional care management, and remote patient monitoring. Coordinate with attending cardiologists for escalation or referral as clinically indicated. Collaborate with AHF attendings and the BSLMC APP team to ensure care continuity, transition-of-care management, and adherence to guideline-directed medical therapy. Participate in clinic operations, including scheduling support, patient education, and documentation within the EMR. In coordination with the BSLMC APP team, independently evaluate advanced heart failure, cardiology and pulmonary hypertension patients. Perform remote patient monitoring and assist with the development of a Transitional Care Management program for patients who have been recently discharged from the hospital. Inpatient Heart Failure Support Provide periodic inpatient HF support to assist with acute decompensated heart failure, pulmonary hypertension, and complex cardiology patients. Conduct comprehensive inpatient evaluation and management of advanced heart failure patients in collaboration with attending physicians. Perform history and physical examinations, order and interpret diagnostic studies, and formulate individualized treatment plans. Manage hemodynamic optimization, diuresis protocols, and device-related care for patients with temporary mechanical circulatory support (MCS) Serve as a clinical resource for nursing staff and house staff on HF-related clinical questions and protocol adherence. Support for Advanced Heart Failure Attendings Assist AHF attending physicians with patient follow-up, including response to MyChart messages, nursing triage inquiries, and longitudinal disease management. Perform pre-operative cardiac clearance evaluations for HF patients undergoing non-cardiac surgery, including risk stratification and optimization recommendations. Manage clinical inbox tasks and ensure timely result review and patient communication on behalf of the AHF attending team. Heart Failure Program Development Collaborate with AHF attending physicians and the BSLMC NP team to support the ongoing development, expansion, and quality improvement of heart failure programs. Contribute to the design and implementation of clinical pathways, HF protocols, and care coordination workflows. Participate in quality metrics review and process improvement initiatives aligned with HF program goals. Assist with the identification of patients into heart failure clinic trials that complement the clinical programs. Minimum Qualifications Master's degree or higher from an accredited Nurse Practitioner or Physician Assistant program.
