Selects and checks case cart and preference cards for correct instrument pan(s), packs, sterile supplies and additional items needed for the case according to the procedure and surgeon preference card. Performs surgical count with circulating nurse according to established policy. 3. Responds to needs of surgeon and other team members. Assists operative team and anticipates needs of the surgeon and/or assistants during cases including the need for special equipment and supplies. Remains constantly observant of the operative field, recognizing emergency situations and performs emergency procedures as directed. Utilizes a variety of OR equipment such as microscopes, CUSA, steam sterilizers, suction, compressed air/gas, etc. 4. Passes and tracks instruments, sutures, and supplies during procedures. Labels all medication and solutions on sterile field. Monitors sponges, needles and instruments throughout the case. Cares for specimens to prevent loss or damage. Uses caution when handling specimens to ensure proper anatomical identification. 5. Assists in completing procedures. Participates in closing counts at appropriate times, sponge and needle count at change of shift according to the count policy. Assists in the application of dressing and/or cast. Assists with transferring patient from OR table to stretcher/bed when appropriate. 6. Prepares operating room and supplies for subsequent cases. Communicates with others to facilitate the timely turnover of operating rooms. Assures all sharps from back table are disposed of properly in biohazard sharps receptacle. Participates in cleaning, decontamination and sterilizing instruments and equipment. Turns over back table and gathers appropriate supplies and equipment for subsequent cases. 7. Position Qualifications: License/Certification/Education: Required: 1. Thorough knowledge of a specialized and technical field of Surgical Technology. Successful completion of an accredited school of surgical technology without having obtained the required surgical technologist certification as of the date of hire, or successful completion of an appropriate program of surgical technology in the Armed services of the United States Public Health Service. 2. New Surgical Technologist (non-certified) employees will, if eligible, obtain a surgical technologist credential administered by a nationally recognized surgical technologist certifying body accredited by the National Commission for Certifying Agencies and recognized by the American College of Surgeons and the Association of Surgical Technologists within one year of graduation as a condition of employment. 3. Current Basic Life Support (BLS) is required upon hire or must be obtained within 30 days of their hire. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. Job Summary: Hours: Per Diem – as needed, during the hours of 8:30am-5:00pm Monday – Friday Union: No Union Name: Non-union Patient Facing : Yes BHCHP is seeking an RN who is interested in flexible weekday hours and working on a small, tight-knit team that provides care coordination services to clients enrolled in the BHCHP OneCare Program. The OneCare program is a state initiative to promote care coordination, continuity of care, and improved health outcomes for those clients under 65 who are eligible for both Medicare and Medicaid. In this role, you will mostly focus on performing Uniform Core Assessment (UCA) assessments and creating care plans customized to meet the needs of individual clients. UCA training is provided by Commonwealth Care Alliance (CCA)/CareSource. You will also help update these plans both annually and when a client has had a significant change in status. You will be required to provide face-to-face outreach to meet with clients, assess their status, and support the client’s progress toward better health outcomes. You will also connect with clients by phone, communicate with providers about client needs, and connect clients to eligible services including visiting nursing, nutrition, durable medical equipment, and social services. Clients may be housed; living in shelters or on the streets and you will need to be resourceful to connect with our clients. It is anticipated that success with connecting with clients will require communication with BHCHP care teams and shelter partners, use of BHCHP and CCA/CareSource electronic medical records, as well as outreach to clients who are living on the street, shelter, or in temporary or permanent housing. You will need to remain flexible and creative, as this role will continue to evolve as experience is gained. There is potential for this role to expand to full-time hours in the future if the candidate is interested. Responsibilities: Assumes responsibility for the coordination of care between BHCHP medical teams and the OneCare program for BHCHP clients who are enrolled in the OneCare Program. Conducts UCA assessments for clients enrolled in the OneCare program; collaboratively participates as a member of the primary care team that includes M.D.s, N.P. /P.A. s, Clinical RNs, behavioral Health Clinicians, Medical Assistants, Case Managers and others. Communicates with clients and/or caregivers including performing outreach as necessary in the greater Boston area for face-to- face contact to homes, hospitals, other facilities, etc. as needed to assess build relationships, client status, and identify new care needs; communicates clearly and professionally with other members of the interdisciplinary team as well as with clients and caregivers. Uses CCA/CareSource and BHCHP’s Electronic Medical Records (EMR) in a timely manner to note all client interactions and outcomes accurately and efficiently; input authorizations for Durable Medical Equipment and other services (Home Health Aide, Skilled Nursing, Physical Therapy, and Personal Care Attendant) based on UCA assessment; bills for UCA assessments into BHCHP’s EMR (EPIC) weekly. Conforms to BHCHP standards of performance around client rights, customer service, infection control, safety procedures and culturally competent care. Remains flexible in duties as role expands and is available for assistance with other projects as needed. Qualifications: Basic Knowledge/Skills: Strong medical and surgical assessment skills. Interest in working with disadvantaged groups; understanding of substance abuse and mental health. Excellent organizational and interpersonal skills; timely and strong communication skills via phone, e-mail, and internal secured messaging systems. Ability to work as part of a team and autonomously; must be able to work with and cooperate with multidisciplinary teams to achieve best patient care. Ability to prioritize and have attention to timetables to complete tasks. Experience with documenting in EMRs: EPIC, Guiding Care, and ECW strongly preferred. Proficient with Microsoft Office Suite and ability to adapt to technological changes. Must be well organized, efficient and can work autonomously and within a team. Driver’s license a plus. Education and Experience: Licensed as RN in Commonwealth of Massachusetts. Previous community health center or primary care experience. 1- 2 years RN experience preferred. Demonstrated interest in public health, primary and preventive nursing and working with an under-served population. Job Description: Position Summary The primary responsibility of this position is to assist residents in Activities of Daily Living. This includes, but is not limited to, toileting, bathing, dressing, taking vital signs and assessing residents’ need for additional medical attention. Nursing Assistants will also provide residents with assistance to take medications, under the careful supervision of the Nursing Coordinator. Nursing Assistants will also participate in the recreation program, under the supervision of the Program Manager. II. Core Care Competencies: Adjust to meet the individual needs of the resident. Position Responsibilities: Assist residents with ADL’s including toileting, bathing, dressing and cueing for feeding. Assist residents with ambulation as well as with mobility devices, including canes, walkers and wheelchairs. Help residents with additional ADL devices, such as glasses and hearing aids. Perform any treatments, as required, including vital signs, weights, temperatures, skin care, and range of motion exercises. Assist residents with medications under the careful supervision of the Nursing Coordinator. Document in, and refer to, all pertinent lists of record keeping systems, ie. toileting book, dietary need book, daily communication book and miscellaneous flow sheets and SAMM documentation.
