As a Full-Time Registered Nurse, Health Center Group Manager , you'll provide high-quality care to client employees and their adult dependents at our health center in New York City, NY , while leading the day-to-day clinical operations of the site. This role offers a consistent Monday through Friday, 8:00 AM to 4:00 PM schedule with no nights, weekends, or major holidays . What You'll Do Oversees the management of single or multiple (2+) health centers and lines of service across contract(s). Manages staff processes typically composed of health center manager, and multiple technical Team Leads as well as indirectly managing all daily operational processes for Medical Directors and clinical providers. Assists in the creation of site-specific goals and measurements related to the needs of the client(s), including management of metrics to Performance Guarantees. Effectively communicates across the various sites and to stakeholders to ensure consistency in messaging. For multiple contract oversight, effectively communicates with DCO to ensure site level priorities are addressed and messaging is consistent across the book of business (BOB). Compiles input and contributes to performance appraisal process for all indirect reports. Responsible for following appropriate processes for hiring, staff development and performance management and provides input to AVP and DCO as needed. Responsible for interviewing, training, and the successful orientation and onboarding of new hires as applicable. Manages daily operations, workflow, work schedules, and timecard monitoring, to ensure efficient and effective health center performance. Identifies process improvement opportunities, workflow efficiencies, determines root cause analysis, creates action plans and presents resolutions and recommendations to the AVP and DCO. Makes recommendations regarding staffing model based on objective scheduling & volume analysis – presents to AVP and DCOs for approval. Understands and complies with all regulatory, procedural and licensing requirements. Oversees the execution of site level quality program and is responsible for supporting and oversight of AAAHC standards across sites. Provides oversight on Member Engagement Site Action plans and outcome reporting. Develops action plans around patient utilization and penetration. Monitors patient flow, scheduling, referrals, and patient satisfaction. Upholds corporate Values in alignment with the Art of Quality Service initiative. Completes reports as needed and provides assistance to Clinical Quality and Operations departments regarding investigations and the resolution of issues or complaints. Coaches and provides feedback to staff on a regular basis. Manages internal budget, reviews expenses, purchases supplies, inventories equipment, medication, lab tests etc. May serve as a client contact and provide reports and summaries of health center activities. Communicates and coordinates Corporate messages and ensures implementation of policies and procedures. Assists in the identification and use of per diem staff to ensure appropriate staffing levels are met. Works collaboratively with the health center managers and Medical Director at the site to manage internal medical staff site issues. Communicates regularly with staff, conducts meetings, and provides updates and shares information regularly. Acts as the health center’s infection control lead and collaborates with the corporate Director of Quality and Infection Control Officer. May provide patient care if qualified and where circumstance applies. May require other duties as assigned. What You'll Bring Bachelor’s degree or equivalent work experience required. Current license as an RN in practicing state. Certification in Occupational Health (COHN/COHN-S) may be required for some sites. Current hands-on certification in AHA or ARC Basic Life Support for health care providers is required. Minimum 3+ years’ experience in the medical field. 2 – 3 years’ management experience. Prior management experience managing a large group a must. Experience accessing CDC, WHO, APIC or other industry standards for Infection Prevention practices. Validates ALC status with finance and any other stake holders identified. 1,3 Maintains a leadership role in the health care team to ensure high quality patient care; identifies health care team educational needs and participates in developing and implementing programs; coordinates multidisciplinary rounds, referrals and case/family conferences. As the clinical lead on the case management triad, maintains ongoing communication with the Social Work Assistant (SWA) to ensure all facets of the discharge plan are completed. This includes off-loading tasks from the SWA’s and completes them. (e.g., Creating referrals for post-acute services, update discharge planning page, arranging transportation etc.). 1,2,3 Ensures participation of patient/family in treatment plan process, including setting realistic goals based on patient's clinical status. Communicates with patients/families to facilitate post-acute care plan. Confirms with family the discharge disposition, date and time for those patients who request it as well as those patients who do not have capacity. 1,2,5 Coordinates patient care with all hospital departments, community agencies and extended care facilities; acts as a liaison with other organizations to assess appropriateness of transfers. Ensures appropriate clinical documents necessary for post-acute placement are included within the referral. 2,3,4 Provides verbal and written explanation of patient’s plan of care, discharge plan, and post-discharge plan of care, encouraging patient and family to participate in plan development; assesses patient and family understanding of all plans . Provides patient/families choice for post-acute providers when indicated. Documents ongoing discharge planning process and discussions in patient medical record. Reviews with the SWA to ensure that discharge planning page in CarePort, is current and accurate for reporting purposes. In collaboration with the SWA, ensures that discharge planning documentation is current and up to date. As the clinical lead, should discuss with SWA when they need support to complete the activities of the day. 1,3,4 Performs monthly chart reviews. Monitors length of stay, timely scheduling of tests/consultations and reports barriers to department leadership for intervention. Participates in Performance Improvement activities, e.g., submits daily discharge stats and weekly statistics. 1,3,5 Identifies, refers and consults with Social Worker, patients with complex discharge problems of domestic violence, sexual assault, child abuse and elder abuse, etc. 1 Participates in developing and implementing clinical practice guidelines; participates on interdisciplinary and external committees. 1,5 Assessment and patient care responsibilities are delivered with knowledge of patient growth and development and are appropriate to the ages of the patients served. 1,6 Maintains established departmental policies and procedures, objectives, performance improvement program, safety, environmental, and infection control standards. 1 Completes any additional ancillary tasks related to discharge planning as needed or directed by case management leadership. 1,6 Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. Skills Proficiency with case management software and EMRs (e.g., Allscripts, Sunrise, or equivalent). Current clinical and technical nursing skills. Knowledge of rules and regulations of child abuse/neglect reporting as appropriate. Knowledge with requirements of regulatory agencies and third-party payors.
