Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Burlington County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care. Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition. Participate in patient care and make nursing visits as needed to support the clinical team. Assist with reviewing clinical documentation for completeness, accuracy, and compliance. Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation. Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families. Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team. Serve as a clinical resource and role model for nursing staff. Participate in interdisciplinary group meetings and other clinical meetings as assigned. Respond appropriately to urgent clinical situations and assist with escalation of patient-care concerns. Qualifications Current, unrestricted Registered Nurse (RN) license in the State of New Jersey. Associate Degree in Nursing required; BSN preferred . Minimum of 2–3 years of professional nursing experience . Previous hospice, home health, palliative care, or end-of-life care experience strongly preferred. Previous leadership, charge nurse, preceptor, or supervisory experience preferred. Current CPR/BLS certification. Valid driver's license, reliable transportation, and appropriate automobile insurance. Strong clinical assessment and critical-thinking skills. Excellent written and verbal communication skills. Strong organizational and time-management abilities. Ability to work independently while functioning effectively as part of an interdisciplinary team. Proficiency with electronic medical records and standard computer applications. Pay Range: $19.32 - $24.13 Position Overview ESSENTIAL FUNCTIONS Prepares patients for examination by performing preliminary physical tests; taking blood pressure, weight, and temperature; reporting patient history summary. Cleans exam rooms, obtains lab specimens. Assists the physicians and other staff as needed. Selects appropriate equipment prior to collection to streamline process. Verifies specimen orders per department policy Prepares exam rooms for patient encounters and assists LIP as requested. Maintains appropriate levels of supplies and all relevant forms for any testing performed and maintains statistics as assigned Performs thoroughly all clerical duties as needed, including but not limited to patient registration, answering phones, scheduling appointments, initiating, maintaining and filing medical records and timely distribution of reports. Performs electrocardiograms and other relevant tests including point of care testing following established policies and procedures and universal precautions. Verifies patient information by interviewing patient; recording medical history; confirming purpose of visit. Documents medical information in electronic medical records including medication and refill information and patient call documentation, keeps rooms stocked and organized. Organizes paperwork and filing. Coordinates with front office personnel and ancillary staff in professional and positive manner in adherence of policies and procedures in support of patient service. Assists in front office functions as assigned including but not limited to: Checkout/in duties with claim creation and assuring complete process of discharge paperwork, referrals, patient education, appointments, insurance eligibility etc. Performs other selected clinical and clerical duties that assist in the delivery of primary health care and patient care management under the direction of a provider and manager in accordance with the objectives and policies of Capital Health (CH) and respective state laws governing such action and activities. Cross-trained to perform Patient Reception Rep job duties as needed. New Jersey Prescription Monitoring Program (NJPMP)- uploads a notarized "request for access" form and a copy of their State-accepted Certified Medical Assistant certificate; performs NJPMP monitoring under the direction of the LIP (Licensed Independent Practitioner). Performs injections, subcutaneous or intramuscular; performs duties in accordance with Capital Health policies. Performs other selected clinical and clerical duties that assist in the delivery of primary health care and patient care management within any CH office location under the direction of a provider and manager in accordance with the objectives and policies of Capital Health (CH) and respective state laws governing such action and activities. Performs other duties as assigned. Incumbents working in Primary Care or Specialty Care offices may be assigned to other Capital Health Medical Group Practices as needed. MINIMUM REQUIREMENTS High school diploma or equivalent. Graduate of a post-secondary medical assisting education program accredited by the National Healthcare Association (NHA), or its successor, The Committee on Allied Health Education and Accreditation of the American Medical Association (CAHEA), or its successor; Accrediting Bureau of Health Education Schools (ABHES), or its successor; or any accrediting agency recognized by the U.S. Department of Education. The educational program shall include, at a minimum, 330 clock hours of instruction and shall encompass training in the administration of intramuscular and subcutaneous injections and instruction and demonstration in: pertinent anatomy and physiology appropriate to injection procedures; choice of equipment; proper technique, including sterile technique; hazards and complications; and emergency procedures. Must maintain current certification from the Certifying Board of the American Association of Medical Assistants (AAMA), the National Center for Competency Testing (NCCT), the American Medical Certification Association (AMCA), the National Association for Health Professionals (NAHP), the National Certification Medical Association (NCMA), or registration from the American Medical Technologists (AMT), or any other recognized certifying body approved by the board. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9.
