Night (United States of America) Department: 100 - 603000 TG - 4N Cardiac Progressive Care Unit Position Description: Under the supervision of the Charge Nurse, the Registered Nurse plans, implements, coordinates and evaluates nursing care for patients. The RN participates in patient and family teaching and provides leadership by working cooperatively with ancillary nursing personnel and other patient team members in maintaining standards for professional nursing practice in the clinical setting. Qualifications 1. Current Licensure by the Louisiana State Board of Nursing is required.

Overview The SOC (Start of Care) Nurse is responsible for providing comprehensive nursing assessments and initiating home care services for patients transitioning into care. The SOC Nurse conducts Start of Care visits, develops and implements individualized plans of care, coordinates multidisciplinary services, and ensures compliance with regulatory, agency, and payer requirements. The role requires strong clinical judgment, care coordination, documentation, and communication skills to support safe and effective patient outcomes. Works under moderate supervision.
