A nursing degree from an accredited college or university is preferred. Minimum of five (5) years of nursing experience required. Minimum of two (2) years of supervisory experience in a nursing care center, hospital, or other healthcare setting required. Brief summary of purpose: The Assessment Nurse Case Manager completes face-to-face home visits for new enrollees within 30 days of enrollment to onboard and completes regulatory assessments.
