Job Details The advanced practice clinician (Nurse Practitioner/Physician Assistant) within the acute care hospital setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APC provides comprehensive care on the inpatient oncology service. Job Description Assessment & Management Performs complete history & physicals and documents in the medical record. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. The APC may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions. Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions. Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. Utilizes evidence-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care. Prescribes diagnostic strategies and therapeutic interventions. May perform advanced procedures consistent with privileges and competency validation. Communication and Collaboration Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family, and staff to promote continuity of care across the inpatient and outpatient continuum of care. Documentation Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and/or clinic note where applicable (H&P, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, death summary) in the medical record per specific patient unit or service. Documentation is timely, meets acute care compliance standards, and captures patient acuity. Professional Practice Demonstrates Professional Practice behaviors including: preceptor/mentoring, education, and instruction of graduate students. Seeks opportunities for active engagement in research and the analysis of evidence-based practice. Actively participates in Advanced Practice Grand Rounds and specialty-specific meetings. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. The Registered Nurse Assessment Coordinator is responsible for the timely completion of all OBRA and Medicare assessments to comply with federal regulations. Oversees the overall process and tracking of Prospective Payment System (PPS) Minimum Data Set (MDS) assessments including completion, submission and correction as needed. Job Responsibilities: Integrates nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure accurate assessments which capture all clinical conditions and provided services. Confirms payer source of all residents per HIQA. Ensures the accurate and timely completion of all MDS Assessments including OBRA and Medicare PPS. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness of MDS and thoroughness of documentation as mandated by federal and state standards. Schedules resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference. Ensures all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B. Assess and evaluates the outcomes of the RAI process to determine additional training, education and monitoring needs Validates that the documentation supports MDS coding. Collaborates with the RAI Specialist(s) and the Interdisciplinary team to identify and capture opportunities to assist residents in reaching their optimal physical and psychosocial levels. Job Qualifications: Education: Graduate of accredited school of nursing. Certificate/License: Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in PA. Work Experience: One year of clinical experience. Experience with MDS completion, reimbursement, clinical resource utilization is highly desirable.
