This role is pivotal to the integrated health team's ability to meet our patients' needs through a unique 24/7 program at the Stout Street Recuperative Care Center. This role will directly support and manage nurses, medical assistants (MAs), and behavioral health technicians. We support medication management, wound care, complex care management, and transitional care for unhoused patients with acute medical needs.
