Job Description The PICU is seeking a Registered Nurse to join our team in Wilmington, DE. This is a 36hrs/wk, rotating shift position. The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Requirements Be a graduate of an accredited school of nursing; BSN preferred. Experienced pediatric intensive care nurse of at least 2 years preferred. Current Delaware or Multistate Compact License Must be CPR certified by the American Heart Association. PALS and ACLS preferred. Job Description Nemours is hiring a Nurse Case Manager . The Nurse Case Manager (NCM)/Case Management RN is responsible for the coordination of care of individual patients in the Inpatient and Emergency Department acute care environment. Through the use of the nursing process, patient care will be assessed, planned, implemented and evaluated with consideration to the appropriate use of resources, anticipatory discharge and timely progression of care. The NCM will manage care with a focus on designated clinical, operational, and financial outcomes for aggregate patient populations. In collaboration with the interdisciplinary team and a family-centered process, the NCM will work to improve outcomes as measured by timely discharge from acute inpatient care, connection to post-discharge care/appointments, family/caregiver access to needed supplies, reduced readmission rates, and improved patient/family satisfaction. Discharge planning, transitions of care and outpatient care of patients within the continuum of care will be aligned with: American Case Management Association Standards of Practice and Scope of Services (ACMA) American Case Management Association Transitions of Care (ACMA) Case Management Society of America Standards of Practice (CMSA) The NCM is accountable for adherence to policies and procedures of Nemours Children’s Hospital, Delaware, and other affiliated hospitals to which Nemours-delegated patients are admitted/seek care. Provides patient/family education on the discharge process, including follow-up needs, and medications/supplies; confirms understanding and documents education as appropriate. Initiates and coordinates referrals to internal and community resources to support discharge needs (e.g., home health, DME, transportation, pharmacy). Ensure timely and accurate delivery and fulfillment of discharge resources. Receive telephone orders from physicians, transcribe orders and record in the electronic health record, and prepare and administer medications and treatments as ordered by the provider. Educate residents and their loved ones to understand the condition, medications, and self-care skills. Promote continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs. Ensure nursing supply and equipment maintenance needs are met. The ability to be a flexible, caring, supportive team member Registered Nurse (RN) Qualifications: Must be currently licensed as a Registered Nurse ( RN) in the state of Delaware or have a COMPACT LICENSE CPR Certification preferred Experience is preferred in rehabilitative or geriatric nursing.
