JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Supervise all aides in performing their duties by checking their work closely to ascertain that assignments have been completed. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Schedule : 44 Hours Bi-Weekly Week 1 16 Hours Mon & Fri 11:00 am - 7:00 pm Week 2 28 Hours Fri 11:00 am - 7:00 pm, Sat & Sun 8:00 am - 6:00 pm Additional hours available as needed based on the needs of the site Clinic Hours: 7 Days/Week 6:00 am - 8:00 pm What You’ll Do Conducts physical exams and preventive health measures per guidelines Performs procedures such as injections, suturing, wound care, and infection management Ensures compliance with regulatory requirements related to clinical operations and manages required physical evaluations (OSHA, DOT, CDC, etc.) Conducts fitness for duty and return to work physical exams as requested Provides medically appropriate care to injured workers using evidence-based guidelines, expedites the return to meaningful work, and collaborates with the TPA and client to quarterback case management Implements and monitors transitional duty programs for injured workers Develops patient care plans, records progress, and ensures continuity of care Educates patients on treatment plans, health maintenance, and development Orders and interprets diagnostic tests to assess clinical conditions Collaborates with physicians and healthcare teams for comprehensive care Prescribes or recommends treatments, including medications and therapies Refers patients to specialists or physicians as needed Maintains patient health records ensure accurate and up-to-date documentation and records Educates and coaches nursing staff on best practices Performs other duties as assigned What You’ll Bring Maintains state RN licenses/certificates to practice Maintains Nurse Practitioner licenses/certificates to practice Maintains State Prescriptive Authority (if applicable) Federal DEA license (matching each state of practice as applicable) State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration) Current national certification (ANCC or AANP required) Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required) Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services Certification in occupational health (COHN/COHN-S/COHN-S/CM) preferred Experience working in an occupational health setting is preferred. Overview Department: Outpatient Wound Care Schedule: FUll-Time, Day Hospital: St. Mary of Nazareth Location: Chicago, IL 60622 Responsibilities The Registered Nurse provides direct care in the inpatient wound care department . The Wound Care RN assesses the skin of patients in the facility, monitors the status of wounds, implements specialized dressing change recommendations based on physician orders and clinical expertise, implements offloading and HAPI prevention orders, adapts to changing policy and procedures according to corporate policy. The Wound Care RN provides ongoing evidence based on relevant education to all facility bedside staff related to HAPI prevention, dressing changes, and care of skin. The Wound Care RN will establish and maintain monthly wound care committee meetings which will include relevant participants from their facility. The wound care RN will collaborate respectfully with all departments to create effective wound care practices based on evidence based best practice guidelines and corporate policies. Communicate with physicians / charge nurse / co – workers, as appropriate regarding changes in the patient’s treatment, clinical condition including results of diagnostic studies. The Wound Care RN will report HAPI occurrence to leadership in a timely manner and has the ability to respond quickly and accurately to changes in condition or response to treatment. Participate in team development calls and meetings. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Must have current and valid license as a Registered Nurse issued by the state.2. BLS upon hire and maintain current.3. Previous Acute Care hospital experience preferred.4. Previous Wound care knowledge and or experience preferred (1 Year). 5. Wound Care Certification within first year of hire. 6. Wound, Ostomy, Continence (WOC) or (WCC) Board Certification preferred. JOB SUMMARY The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility. SUPPORT DUTIES Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff. Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training. Encourage family involvement through volunteering and visitation. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach. Be flexible to meet the change in residents’ needs. We need to adapt to their needs rather than expecting them to adapt to our needs. Provide emotional, caring support to residents, families, and the inter-disciplinary team.
