Job Summary The Clinic Manager (CM) is an essential part of the collaborative patient care team that supports an exceptional patient experience. The CM is empowered to practice and contribute throughout the care experience. Implements Crossover’s standards of care and operations workflows. Provides patient care, using Crossover’s patient-centered medical home approach, including phlebotomy services and immunization services Resolves customer complaints; assists customers with inquiries in connection with clinic services and hours of Operations. Handle inventory, orders, and replenish medical supplies and materials. Co-leads clinic-wide huddles, team meetings, and ensures the clinic team is kept well informed of corporate and client communications, which includes clinical practice updates. Manages the schedule for efficiencies and optimal staff coverage, including PRN talent pools. Hires, coaches, and develops staff. Assists in onboarding and training new team members. Provides guidance and direction to care team members, including nurses, hosts, operations leads, and care navigators (site specific). Partners with the Triad and multidisciplinary care team to develop plans for maximizing client outreach and increasing clinic utilization. Identifies and recommends actions to enhance current services, partners with the Triad on developing and implementing new services. Hires, coaches, and develops staff. Assists in onboarding and training new team members. Encourages continuing professional education and development to enhance learning and meet licensing expectations. Provides guidance and direction to care team members, including nurses, hosts, operations leads, and care navigators (site specific). Leads and facilitates any event that supports client goals and member health strategies. Collaborates with various departments to ensure the clinic is compliant with regulatory bodies. Assists in performing qQuality and cCompliance checks required on a daily, weekly and monthly basis Contributes to reporting requirements related to incident reporting and compliance Act as the first point of contact for IT questions and issues. Coordinate and assist internal TechOps with support requests. Performs other duties as assigned Required Qualifications Graduated from an accredited nursing program Active nursing license (RN) Minimum 3 years of clinical experience with demonstrated strong clinical leadership in similarly sized operation Current BLS (Basic Life Support) certification Minimum 3 years experience opening, operating and managing a multi-speciality, multi-provider clinic, or equivalent Minimum 3 years of experience staffing and managing a team of employees Minimum 3 years of experience utilizing health analytics information. Certified Nursing Assistant (CNA) needed in Brooklyn Well-staffed Nursing and Rehabilitation Center 3pm-11pm JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident.
