Purpose: We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and complex care patients in the Philadelphia, PA region on a full-time basis. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases. What You'll Do: Provide high quality care to primary and palliative patients in and around Philadelphia, PA. Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care. Registered Nurse (RN) – Hospice Case Manager Location: Lehigh Valley, PA (ZIP Code 19518 and surrounding areas) Schedule: Full-Time | No Nights, No Weekends Position Summary We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Lehigh Valley area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.
