Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. To monitor the performance of CNAs/GNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care. This position will oversee the Bergen/Essex/Union county area. The hours are Monday-Friday 11am-7pm Responsibilities Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals. Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team. Complete all required admission documentation accurately and in a timely manner. Develop and initiate individualized plans of care. Educate patients and families on hospice philosophy, services, medications, and expectations. Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers. Obtain informed consents and ensure all regulatory requirements are met. Collaborate with clinical staff to facilitate seamless transitions of care. Maintain compliance with Medicare, state, and agency regulations. Participate in on-call or weekend rotation as assigned (if applicable). Provide compassionate emotional support to patients and families during the admission process. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of practice. Minimum of one (1) year of hospice nursing experience required. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support.
