Job Description Inova Schar Cancer Institute’s Fairfax location is looking for a dedicated Oncology Nurse Navigator to join the team working in their Saville Center for Cancer Screening and Prevention with a focus on Cancer Diagnostics. This role will be full-time day shift from Monday – Friday. Our Navigators work in an exempt capacity, but we anticipate the schedule to be Monday – Friday with regular day shift hours. Our Fairfax clinic is not open on weekends or Inova-recognized holidays. The Oncology Nurse Navigator provides safe therapeutic care in a holistic and systematic way across the health system and continuum of care and in the community for past, current and potential cancer patients. Guides patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care. Acts as a liaison, advocate and point of contact to patient/family members by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services. May assist patients and families with resolving financial, psychosocial, functional, and administrative issues by advising of options and referring to appropriate resources. Acts as advisor/educator by providing emotional support, counseling, clinical education, resources and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making. Facilitates and oversees the care delivery for patient under the direction of the department manager. Job Responsibilities Anticipates patient's needs and advocates for patient rights and appropriate utilization of services. Builds working relationships with patient, healthcare providers, and community resources to maximize patient outcome and identify/resolve barriers to the plan of care. Conducts ongoing promotion of healthcare program to community-based resources to include developing written materials and public presentations describing activities related to patient care and research. Coordinates community health programs and cancer-related education and groups. Conducts comprehensive assessment of the patient's health and psycho-social needs, and develops a case management plan in collaboration with the patient and their support system. Assesses patient needs and coordination of multidisciplinary care. Serves as a clinical consultant to the health care team. Coordinates a patient's plan of care which may include answering patient questions regarding disease process and plan of care, appointment scheduling when appropriate, assisting with referral process, and providing community resources. Coordinates patient discharge from hospital to insure adherence to SOPs. Maintains a working knowledge of availability of community resources and financial services for providing clinical care and knowledge about appropriate referrals. Educates patients and family members regarding disease process. Completes documentation related to pretesting, plan, written consent, orders and outcome according to policy; communicate verbally or in writing to other team members. Department 10307 Pediatric Intensive Care Unit Pay Range $33.08 - $60.18 Job Description Summary $25,000 Bonus with a 3-year Commitment Day Shift / Night Shift Rotation Weekend and Holiday Rotation Who We Are The Pediatric Intensive Care Unit (PICU) is a 46-bed unit. We will be expanding to 60 beds in 2027! This unit serves as a regional referral center and is a designated Level I Trauma Center for comprehensive care to pediatric patients and their families. The PICU Patient Care Management Team coordinates interdisciplinary patient assessment and patient care and provided by pediatric intensivists and specialty nurses in conjunction with multiple specialty services within the Medical Center. Patients are admitted to the PICU from the ED, Trauma Services, the Operating Room, and transfers from in-house units as well as transports from other institutions via the Pediatric Transport team. Trauma patients comprise about 10%-15% of patients in the PICU. These patients often have multiple injuries or severe head trauma. The PICU is also a Pediatric Burn Center and receives referrals from the region. Other complex patients requiring general surgery, ENT, craniofacial, neurosurgical and other surgical subspecialties are also cared for in the PICU. Medical patients comprise 50%-60% of patients and include children with respiratory failure, meningitis or seizures, sepsis, multi-system organ failure and chronically ill children. Children with cancer also require PICU care for complications that develop as part of the underlying disease, chemotherapy or following bone marrow transplantation. Services are provided daily, 24 hours per day. The Division of Critical Care Medicine (CCM) provides services and care to all patients admitted to the Pediatric Intensive Care Unit (PICU). Specifically, care is provided for all patients in the PICU, patients within the hospital by request, patients within the hospital as part of all emergencies and resuscitations, and patients in the ED for “medical alerts” and “trauma stats”. The scope of services includes resuscitation, life support, monitoring and treatment for life threatening events and diagnostic issues related to these items. Patients may be admitted to and discharged from the PICU based on pre-established guidelines. See section on critical care units. The department utilizes a multi-disciplinary approach to care, utilizing the services of physicians, nurse practitioners, nurses, respiratory therapists, social services and other support services necessary to provide quality care. Attending physicians providing care are board eligible or certified in Pediatric Critical Care. The PICU provides CAT (Rapid Response Services) and Code Blue team leadership to support in house clinical emergencies. An attending or fellow along with an ICU nurse and Respiratory Therapist respond to assist in clinical rescues of in-house patients. This service can also be activated by a concerned parent.
