This would include contacting insurance companies for information needed to facilitate discharge. Assess the health status of assigned patients by collecting and analyzing patient data and evaluate appropriateness of nursing, medical and ancillary care. Evaluate potential for safe discharge to home, identifying limitations and need for referral services. Thoroughly and accurately document the discharge plan ensuring regulatory compliance. Tracks faxes sent to insurance companies to ensure certification information is received. Reviews the EMR when further information is needed by insurance companies and follows up to make sure that information is submitted in a timely fashion.
