The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-services. Fulfills the obligation of requested assignments. Performs other job-related duties as assigned. POSITION QUALIFICATIONS : Understands and is committed to the hospice philosophy of care. To support our staff are Clinical Nurse Managers (CNMs) which help with patient care, through put, and assignments. Nurses who are staffed on Kelemen 9 are required to have Advanced Cardiac Life Support (ACLS), and Basic Life Support (BLS) certifications, which will allow competent and safe care for our complex patient populations. POSITION SUMMARY: Under the supervision of a licensed RN or LPN performs nursing services based on the facility’s policies and procedures as regulated by the State of NJ, DOH and Federal regulations. Provides assistance to residents with daily routines and other resident activities as needed. PART I: JOB RESPONSIBILITIES AND STANDARDS 1. Follows the plan of care. Attends IDCP meeting as assigned. 2. Demonstrates proper body mechanics. 3. Provides personal care to residents/guests (bathing, cleaning finger nails, shaving and perineal care, etc.) and assures that resident/guest dresses appropriately. 4. Follows infection control procedures (i.e. hand washing/sorting of linens) 5. Performs routine /special skin care as indicated. Reports skin alterations immediately to nurse. 6. Provides privacy for residents and adheres to HIPAA standards. 7. Inquires about residents/guests comfort/discomfort level on a scale of 0-10. 8. Provides restorative care as indicated. 9. Performs routine catheter care 10. Provides care to residents/guests with tubes and special equipment. 11. Transports resident/guest to and from activities. 12. Answers all call bell and places them in reach of resident/guest. 13. Fills water pitchers with fresh ice water unless contraindicated. 14. Offers snacks to all residents/guests and documents appropriately. 15. Monitors residents during smoking periods when assigned. 16. Demonstrates ability to deescalate resident through verbal strategies, redirections and crisis management and report to nurse immediately. 17. Assist with meal pass and feeding in a timely manner. 18. Follows Fall Prevention Protocol. 19. Collects lab specimens. 20. Reports condition changes/behavioral issues to nurse immediately. 21. Follows elopement risk protocol. 22. Maintains cleanliness and organization of resident/guest rooms including bed making. 23. Accompanies resident/guests during outside medical consultations and out of building activities. 24. Follows safe-handling procedures for personal safety and resident safety at all times. 25. Overview The Children’s Regional Hospital at Cooper University Hospital is a pediatric facility that consists of several units that meet the diverse needs of pediatric patients: Pediatric unit, Pediatric Intermediate unit (PIMU), Pediatric Kostic Infusion Room (PKIR), and Pediatric Intensive Care unit (PICU). We provide care for pediatric patients aged 0 to 18 years. We offer subspecialty services through consultation and referral in areas such as Endocrinology, Genetics, Infectious Diseases, Neurology, Neurosurgery, Gastroenterology, Otolaryngology, Pediatric Surgery, Trauma, and Hematology. The Family-centered rounds are conducted daily with an interdisciplinary care team including physicians, professional nursing services, child life specialists, social services, and nutrition support. All our pediatric units are secured with restricted access for staff, with visitors gaining entry via monitored access. All pediatric patients wear security tags to assure their safety. Our facilities include a teen room, a spacious playroom for younger patients, and a family waiting room. Our Pediatric unit and PIMU have 38-bed capacity. The Pediatric unit is in Kellemen building on South 7th and offers 26 beds. Experience Required 1 year of RN pediatrics experience is preferred. Education Requirements ASSOCIATES License/Certification Requirements Current NJ-RN License (Registered Nurse). Current NJ-BLS Certification (Basic Life Support).
