Registered Nurse - Job Description FLSA: Non-exempt POSITION SUMMARY: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing policies and procedures. TITLE OF SUPERVISOR: Unit Manager / RN Supervisor TITLE OF IMMEDIATE SUBORDINATES: Certified Nursing Assistants POSITION REQUIREMENTS - Education / Experience Registered Nurse licensed in the State of Massachusetts. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred. CPR certified. Ability to read, write and comprehend English; ability to follow oral and written instructions. Fluent knowledge of the MS Office suite and other office equipment. Ability to work hours as scheduled based on the requirements of the position assignment. Working Conditions: Works in an appropriately lighted and ventilated environment. Physical Requirements: Must be able to move intermittently throughout the day, proper body mechanics required . Ability to perform physically demanding work involving frequent bending, stooping, turning, stretching, and reaching above the shoulders are involved. Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately to assure that the requirements of this position can be fully met. Must be able to lift a minimum of 50 lbs. In addition, this role focuses on performing the following duties: Functions as part of the health care team in providing safe and effective therapy for assigned patients under the direct supervision of a registered nurse. Participates in the implementation and evaluation of patient care. Requires an LPN license. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices. Job Overview The position provides a supportive role to the clinical management team and performs a variety of duties in the coordination of the delivery of care to patients and families in their homes. Triages calls related to patient care, including physician orders, staff issues and other clinical issues. Job Description Minimum Qualifications: 1. Licensed Practical Nurse (LPN). 2. Three (3) years of community health experience required or combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience required. Preferred Qualifications: 1. Prior experience in telephone triage preferred. Communicates regularly with Clinical Managers as well as with the Clinical Director when issues arise relative to clinical practice or other performance related issues. 4. Demonstrates expertise and knowledge in a variety of areas, including, but not limited to: Current methods of care and treatment; Policies, procedures and protocols which are specific to the Agency; The Agency Infection Control Plan; A working knowledge of all reimbursement criteria; A working knowledge of all regulations; Has a working knowledge of the Medicare Conditions of Participation; and Assisting with individual caseload reviews or other audits as assigned by the manager or Clinical Director. 7. Assists with orientation and preceptor of clinical staff. 8. Demonstrates the clinical expertise needed to provide effective guidance to clinical staff related to patient visit frequency, skill and homebound status, and patient interventions. 9. Maintains a calm and professional demeanor and customer service-oriented approach during all interactions with patients, patient family members, and colleagues. 10. Works collaboratively with all disciplines, facilitating productivity, cooperation and efficient operations. 11. Assists in clinical manager’s and scheduling coordinator in appropriate coverage based on census, skill mix, geographic location, patient need and clinical capacity. 12. Triages calls from physician and follows through on required actions, including communication to field staff, changes in orders, scheduling prn visits, and f/u as clinical resource to clinical staff if needed. 13. Utilizing voice mail and e-mail, communicates with others, as necessary in an effective and efficient manner. Responds to inquiries in a timely manner. Documents appropriately. 14. Consults with Managers regularly, providing information about progress and concerns. 15. Provides documentation which is concise, accurate and informative, and which supports reimbursement. 16. Assists with reviews of clinical reports such as timelines of care and other audits as assigned by the Clinical Director. 17. Represents the Agency at community and/or professional organizations. 18. Practices in accordance with standards of care, policies, procedures, and scope of practice, demonstrating an awareness of regulatory guidelines. 19. Demonstrates a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise. 20. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Tufts Medicine Care at Home policies and procedures. 21. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines. 22. Participates in all mandatory in-services. Physical Requirements: 1. Prepares patients for examination and treatment in a timely manner , including: obtain vital signs, urinalysis and blood glucose, microalbumin, and A1 C. Obtain, document and reconcile allergy and medication lists. Collect and prepare specimens and relevant documentation for transportation to off-site laboratory, assuring accuracy in labeling and documenting . 2. Assists with any office examinations/procedures/tests/treatments. Administers appropriate injections to patients of all ages and draw blood specimens, following office & CDC guidelines. Maintain patient education resources and provide instruction and education to patients as instructed by providers. 3. Anticipate necessary labs, reports and consults the provider may need. Reconcile all lab results in EMR. Send out patient reminder letters and maintain patient compliance scheduling. 4. Prepare and replenish medical supplies including vaccine inventory and monitor medication refrigerator temperatures based on DPH requirements. Follow DPH protocols regarding safe keeping of all medications and vaccines . 5. Clean and sterilize instruments and maintain diagnostic equipment. Quality control and record on all in-house testing i.e. urine, strep, blood glucose monitors and pregnancy controls. Report any malfunctions to BioMed. Arrange for any replacement / upgrades to all medical devices. 6. Facilitate daily operations and responsibilities of the day-to-day patient flow within each designated area, acting as the first level “go-to person” for the providers, clinical and clerical staff regarding troubleshooting issues such as patient complaints, billing questions, registration issues 7. Assists with provider schedules and staff schedules . Coordinate and/or provide appropriate coverage for vacation requests and sick calls . 8. Acts as Electronic Medical Record “Super User”, attending regular EMR training sessions, distributing new information or workflows to providers and staff and training of new staff members on EMR . 9. Participates in the interviewing, hiring and performance evaluation of staff, providing on-going useful, objective and constructive input. Facilitate the orientation of new and per-diem staff, including: creating orientation schedule and training of both clinical and clerical duties. 10. Works to foster and facilitate a respectful and collaborative relationship between nursing team and MA team in order to successfully meet site specific and organizational goals. 11. This position will act as a “subject matter expert” in the roles, duties, scope of practice, and work flows for other staff members . Physical Requirements: 1. Knowledge of computer systems and software (Microsoft Office) with ability to learn and retain new applications. 5. Ability to work under stressful conditions, adapting to the changing pace on the unit, frequent interruptions, emergencies and critically ill patients. 7 .
