Position Summary Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions. Provides clear explanations of hospice philosophy, services, and the admission process to ensure understanding and promote a smooth and supportive transition to hospice care.
