ESSENTIAL DUTIES & RESPONSIBILTIES: By following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following: • Maintain a healthy, safe environment. Providing nutritional and environmental support. • Assist with personal hygiene, toileting, dressing, walking and feeding the client. • Position the client when instructed to do so. • Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor. • Remind the client to take medications. • Assist with payment of bills ONLY when authorized to do so and it is documented on the assignment sheet. • Run errands when authorized to do so. Documentation on the Aide Charting Sheet daily. • Report any important information or changes in the client's condition to the agency and/or the nursing supervisor when necessary. • Use of medical supplies and equipment when appropriate. • Emotional support and the development of a trusting relationship between the aide and the client. • Encouragement of client independence. QUALIFICATIONS/EDUCATION: • Must be eighteen (18) years of age or older. • Must have a certificate from a Home Health Aide training program consistent with State requirements with evidence of ongoing in-service education to maintain the certificate. • Must have the maturity necessary for establishing and maintaining a good work relationship with the client, the client's family and agency personnel. • Have a sincere interest in and a sympathetic attitude towards caring for the sick and disabled. ESSENTIAL SKILL/KNOWLEDGE FUNCTIONS Assists patients in and out of bed and into wheelchair with the correct use of the transfer belt. Assists patients in bathing; to include bed baths, tub baths, and showers. Assists patients in dressing or undressing. Assists patients in using bathroom and/or bedpan and peri-care. Takes vital signs of patients, e.g., temperature, blood pressure, and respiration and records information. Assists patients to dining area for meals. May be required to assist in feeding of patients and documenting intake of nourishment. Strips bedding and replaces with clean linens. Assists in personal grooming of patients, e.g., brushing hair, trimming fingernails and toenails, and shaving. Turns patient intermittently of patient is bedridden due to illness. Required to clean bedpans and empty catheter bags. Documents and/or reports information regarding the patients’ health, I&O, any change of medical condition, and care given. Distributes fresh water to patients and picks up used water pitchers. Required to pass food trays to individual patients in accordance with diet cards on trays. Provides oral care and denture care. Practices proper body mechanics while moving/transferring patients. Provides range of motion exercises for patients. Collects stool and urine samples. Recognizes and reports signs/symptoms of abuse and/or change in condition. Properly documents in accordance with established guidelines. Receives and gives reports on patient’s status or condition. Assists patients to and from activities as requested. Assists patients with walking (with or without assistive devices). Provides care for the dying patient. Provides post mortem care as requested. May be subject to hostile and emotionally upset patients, family members, associates and visitors. OTHER CONSIDERATIONS AND REQUIREMENTS In this position, the associate is required to be on his or her feet continually throughout the day. In a full-time nonrestrictive basis, the associate must be able to lift at least 40 pounds of weight, which is required when transferring a patient from a bed to a wheelchair. A transfer belt is necessary and required when transferring any patient. This is for the safety of not only the patient, but also the aide. Other nurses are available to assist with heavier patients as needed. Follows up with unit managers on issues/concerns noted during review to ensure appropriate nursing follow up occurs. Reviews nursing schedules to assure they meet resident/facility needs. Participates in the recruitment and selection of nursing personnel. Development and delivery of in-service education to nursing staff. Utilizing education to expand the knowledge, skills and confidence of the nursing staff in their required job applications leading to compassionate, quality care and respect for resident rights. Proactively develops positive employee relations, teamwork, mutual respect and effective communication among staff. Assists in conflict resolution of staff when indicated. Monitors staff for compliance with OSHA mandates and facility policies on workplace safety. Proactively develops procedures and incentives to promote workplace safety and safe work practices. Establishes implements and monitors the infection control program designed to provide a safe, sanitary and comfortable environment designed to prevent the devilment and transmission of disease and infection. Reviews infection control surveillance reports to identify trends and to develop effective actions to control and prevent infections. Maintains and compiles infection control reports and statistics for the facility, reporting information to the Director of Nursing and government agencies as appropriate. Actively participates as a member of the QA Committee. As designated by the Director of Nursing; participates in monitoring nursing services for compliance with standards and regulatory requirements through rounds and auditing of nursing operations with immediate remediation of inappropriate practices. Compiles information obtained to summarize findings for the Nursing Director to review. Helps the Administrator prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc. Reviews and reinforces important standards previously cited. Participates in the development and implementation of a Survey Preparedness program in anticipation of an annual inspection survey. Participates in the development and implementation of a Plan of Correction response to an inspection survey and any follow-up QA required for nursing. Communicates directly with residents, families, medical staff, nursing staff, interdisciplinary team members and Department Heads to coordinate care and services, promote participation in care plans and maintain a high quality of care and life for residents. Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from residents or families about nursing services. Participates in investigations for allegations of potential abuse, neglect, mistreatment or the loss/ misappropriation of resident property as requested by the Director of Nursing. Promotes compliance with accident prevention procedures, safety rules and safe work practices to prevent employee injury and illness and control worker’s compensation costs. Assures all staff are appropriately trained in fire, disaster and other emergency procedures. Actively participates in the evaluation of staff response and performance during drills. Position Summary To fulfill the needs of Diagnostic Imaging patients in a variety of age groups, their family members, and the community by performing varied patient care tasks and functions in accordance with licensure, certification, training, and regulatory requirements. Education Bachelor`s degree in Nursing required. Current/valid State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing). Completion of general nursing orientation, CPR, ACLS and PALS certification and unit-based orientation. Schedule: Shifts are 8 hours every weekend Saturday and Sunday, varied start times (7am to 8:30 am) based on the needs of the department. Experience Computer and keyboard skills. Medical terminology required. Cat Scan and MRI experience preferred but will provide unit-based training to qualified candidates. Radiology experience preferred; however, willing to train experienced applicants . Skills Physical and cognitive requirements: Position's physical requirements necessitate a full range of body motion including but not limited to, physically assisting patients, requiring stretching, bending, lifting, sitting, kneeling, stooping, standing, walking, pushing and pulling. Position is classified according to the USDOL as Very Heavy Work which is defined as exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. The National Institute of Safety and Health (NIOSH0 guidelines, which recommends that the average worker not lift more than 51 pounds without assistance, will be followed. Manual and finger dexterity and hand-eye coordination are also required as is constant talking and listening in English. Must demonstrate effective oral and written communication skills, including speech clarity, understanding the information relayed, deductive and inductive reasoning skills (the ability to apply general rules to specific problems to produce answers that make sense and the ability to combine pieces of information to/from general rules or conclusion respectively). Considerable walking, standing, sitting, reaching, filing, typing and photocopying required. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment related to changing needs of patients with acute, chronic and complex and/or terminal disease processes. Ability to work cooperatively within health system, multidisciplinary team members (i.e. MD's, Rehab, Respiratory, DI, Laboratory, etc). Ability to utilize effective time management skills to set priorities to perform responsibilities, problem solve and deliver care.
