Responsibilities: A Registered Nurse who, under the direction of the nursing leadership is responsible for planning, directing, implementing, coordinating, and evaluating nursing care for a specific unit during her/his shift. Receives report from previous shift, making sure that he/she has received all pertinent information on each resident. Continuously evaluates behavior, symptoms, reactions and progress of residents and discusses findings with Nurse Manager/Charge Nurse. Administers and documents all medications according to established policies and procedures, physician's orders and Department of Health regulations. Performs all treatments according to physician's specific order and established policies and procedures. Discusses the need to discontinue or change a medication of treatment with the Nurse Manager/ Charge Nurse. Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy. Qualifications: Current NY State Registered Nurse License Education: Associate Degree BSN Preferred

Position Summary The Registered Nurse (RN) is responsible for overseeing and delivering quality nursing care to residents while ensuring compliance with all federal, state, and facility regulations. The RN will supervise nursing staff, collaborate with physicians and interdisciplinary teams, and advocate for the highest standard of resident care. Essential Responsibilities Assess, plan, implement, and evaluate resident care plans. Administer medications and treatments in accordance with physician orders. Monitor resident conditions and promptly communicate changes to physicians and family members. Complete accurate and timely nursing documentation within the electronic medical record. Supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). Ensure compliance with infection prevention and control standards. Respond appropriately to emergencies and changes in resident condition. Participate in admissions, transfers, and discharges. Collaborate with rehabilitation, dietary, social services, and other departments to ensure comprehensive resident care. Educate residents, families, and staff regarding care plans and health conditions. Maintain compliance with New York State Department of Health regulations and facility policies. Participate in quality improvement initiatives and staff education programs. Qualifications Required: Current New York State Registered Nurse (RN) License. Graduate of an accredited nursing program. Current CPR/BLS Certification. Excellent communication and organizational skills. Ability to work collaboratively in a fast-paced healthcare environment. Certified Nursing Assistant (CNA) needed in Brooklyn Well-staffed Nursing and Rehabilitation Center 3pm-11pm JOB DESCRIPTION: The Certified Nursing Assistant works directly under the supervision of the Registered Nurse or Licensed Practical Nurse (LPN) and performs duties in a relation to the personal needs of the resident, care of supplies and equipment, and maintaining the cleanliness, comfort and safety of the resident in his/her environment. REPORTS TO: Charge Nurse, Supervisor and Director of Nursing Services (DNS) Ability to comprehend and follow verbal and written instructions. Ability to write legibly. Physically able to perform the required duties of a Certified Nursing Assistant. Experience as a Nursing Assistant in an acute care or long-term care setting. RESPONSIBILITIES: Residents Provides or assists with bed bath, tub bath, whirlpool and showers. (Supervises safety of same.) Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Safety of Residents: Restraints are to be properly applied and released and removed every two (2) hours for at least 15 minutes. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques.
