POSITION SUMMARY As part of a multi-disciplinary team providing medical care, the PCA is responsible for patient intake in the clinic and front desk, obtaining complete medical history, provides education about human sexuality, family planning, abortion, colposcopy, LEEP and general health education, as applicable. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Front Desk: Patient Reception and Visit Flow (15%) • Monitor health screenings at front desk. • Greet all patients and visitors according to PPGNY standards and collect all pertinent demographic and billing information, including insurance verification, following PPGNY and NYSDSS guidelines. • Change and update patient demographics in electronic health record. • Initiate CVR (superbill) and activate electronic health record to start visit. • Sign appropriate consents and reception forms. • Manage walk-in clients according to availability in the schedule. Scheduling, Appointment Schedule Management, Phones (15%) • Answers phones and triages patient calls, including appointment management. • Scrub the schedule and prepare charts for next day services according to information required by visit type and services requested. Billing and Revenue Cycle Duties (10%) • Process supply requests, collect payments and co-pays in accordance with PPGNY supply sales, cash handling, and banking policies. Department/Unit: HBD - General Pediatrics Work Shift: Day (United States of America) Salary Range: $83,200.00 - $128,960.00 RN – Ambulatory Under the direction of the practice physician and/or advanced practice provider (APP) and the supervision of the Nurse Manager (RN) and/or Nurse Supervisor (RN), the RN may provide direct patient care, patient triage (in-person and telephonic), assessment, planning, directing and evaluating of a patient’s specific care plan and clinic visit. Able to work effectively within a team of multi-specialty professionals Job Description Obtain and accurately record patient’s vital signs; reports abnormal values to treating provider Obtains and documents a patient’s history (Medical, Surgical and Social) and focused physical assessment, related to the clinic visit and as dictated by clinic procedure Administers medications and treatments to patients in clinic, under the direction of a physician or APP Participates in in-office procedure, as related to clinic specialty, under the direction and supervision of a Physician or APP and documents results in EHR Performs review and triage of incoming test results, patient requests and pharmacy renewals; facilitates call-backs to patients as necessary Facilitates the utilization of resources to meet patient outcomes and contribute to Facility (AMC), New York State and National Quality data metrics Delegates tasks deemed appropriate per licensure of LPN and/or MA Adheres to AMC’s regulatory agency and internal compliance policies Orientation and education of newly hired staff Completes continuous/annual education related to Clinic specialty to maintain knowledge base on current practices Provides education to patients related to their visit and overall health and wellness Minimum Qualifications Associate’s degree of a state accredited Registered Nursing program is required NYS current RN licensure to practice professional nursing is required Current CPR certification required Certification in specialty, Ambulatory setting preferred Excellent customer service and communication skills Ability to effectively present information and respond to questions from physicians, patients and their family members or other employees within the work setting . Familiar with computer software. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. Responsibilities Roles & Responsibilities: CLINICAL PRACTICE Assesses the health status of patients/families by means of comprehensive health history and physical examination. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. Documents findings and plan of care in the patient's medical record. Consults with physician according to established standards, policies, procedures, and protocols. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. Initiates referrals to specialty services and fosters continuity of care. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. Maintains patient/employee confidentiality in the management of information. Develops the plan of care based on the diagnoses and individual patient needs: • Records, reports, and interprets the client’s responses to the plan of care. 1 Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. Demonstrates accountability for own nursing practice. Advocates for patients/families and supports patients' rights. Considers ethical issues of professional nursing practice and adheres to the code of ethics. Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. Serves as a resource person to nurses and other health team members. Participates in peer review. Incorporates research findings in clinical practice. Participates in approved nursing and medical research studies. Keeps knowledge current by self-directed learning, literature review and attendance at educational programs. Participates in defining, maintaining, and interpreting standards of nursing practice. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. Seeks opportunities to enhance professional nursing practice and influences outcomes. Treats co-workers, patients and families with dignity and respect. Qualifications Requirements: Master?s from an accredited Nurse Practitioner Program Current NYS Nurse Practitioner and Registered Nurse licensure and certification. Current Board certification as an NP. ONS/ONCC Chemotherapy Immunotherapy certificate required BLS and ACLS required Previous progressive experience relevant to the area of clinical practice preferred Demonstrates a professional, courteous, and respectful attitude, Excellent technical and patient care skills. Excellent communication skills, written and verbal. Working knowledge of personal computers and some prior hands-on experience Models a customer service focus. Respects patient confidentiality, privacy and dignity. Requires appointment to Medical Center Allied Health Personnel Staff.
