Qualifications EDUCATION Graduate of an accredited school of nursing; BSN Preferred. Proficient in use of computer software to include electronic patient records. MDS Coordinator The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement. Responsibilities: Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process. Coordinates and facilitates care conferences with the interdisciplinary team. Coordinates, completes and reviews Minimum Data Sets (MDS) Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS. Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced. Will cross train between PPS and OBRA. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, TJC, and State & federal regulations. Reports to Nurse Manager. Clinical Practice- On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. Population Specific- Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Unit Leadership- Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. Documentation/Communication - Completes all required documentation in a timely manner, according to CCMC policies and standards; necessary information is effectively communicated to all members of the health care team. Patient/Family Partnership & Education- Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. Education and/or Experience Required: ASN required upon hire. BSN preferred upon hire and required within 3 years of hire. Minimum of 2 years clinical experience on Medical/Surgical unit; PACU and/or pediatric experience preferred. Endoscopy Nurse requires knowledge of surgical specialties relative to procedures and equipment. License and/or Certifications Required: Current State of Connecticut Registered Nurse licensure required and maintained thereafter. Current Basic Life Support (BLS) / CPR in adult, child & infant is required upon hire. Advanced Cardiac Life Support (ACLS) required within six months of hire. Specialty licensure/certification may be required, as specified by department. Position Specific Job License and/or Certification Required: Pediatric Advanced Life Support (PALS) Required within 6 months of hire or before staff has completed dept. orientation if this is longer than 6 months. Develop an individualized perianesthesia plan of care by implementing: C ommunicating the patient’s current status throughout the perianesthesia continuum of care. Evaluating the patient’s past medical history and surgical plan using the PAT anesthesia protocols Interviewing pre-operative patients to gather necessary data to assist for admission Reviewing all preoperative test results and notifying physicians when results are outside normal limits or incomplete. Serving as initial and primary contact for referral sources to direct screening and clearance of patients scheduled for, or potentially to be scheduled for procedures. Understanding medications and their actions and educating the patient on which medications need to be taken day of surgery. Demonstrating safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation. Speaking up with safety concerns acting as the patient’s advocate. Handling specimens according to facility policy and procedures. Adhering to standard precautions including the use of personal protective equipment. Qualifications Graduate of an Accredited School of Nursing. One year of pre-op, PACU , and/or medical / surgical nursing experience required. Current state licensure to practice within good standings. Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 days of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advanced Life Support (PALS) course is required when pediatric care is included in the facility scope services within 90 days of employment or documentation of current PALS certification.
