Livingston Hills Nursing and Rehabilitation Center, a 120 bed Skilled Nursing Facility, located in Southern Columbia County, is looking for dedicated, caring, and knowledgable Licensed Practical Nurses to join our Care Team! We are offering a generous sign on bonus for any LPN hired for full and part time! DUTIES: Collecting required information from new Residents to be admitted. Recording health details of Residents; including vitals & temperature. Administering medications and injections to Residents as needed. Treating and dressing wounds as needed. Supervise and support Certified Nursing Aides (CNAs). Helps Residents get dressed & take care of personal hygiene. Monitors Residents’ food and liquid intake and output. REQUIREMENTS: Valid LPN license Must be in good standing with State Registry Must be able to work as a team member 
Livingston Hills Rehabilitation & NCC is a 120 bed skilled nursing facility located in Hudson, NY. Our residents are both short term rehabilitation and long term care residents. We are looking for fun, energetic, motivated, dedicated and dynamic staff for our recreation department. As a Nurse Practitioner with Integrated Wound Care, you will play an instrumental role in making a positive impact on the lives of our patients. Responsibilities Conduct wound assessments and provide personalized treatment plans for patients Work collaboratively with a team of healthcare professionals to ensure optimal patient care Complete documentation accurately and in a timely manner Educate patients and their families on wound care, prevention, and management Monitor patient progress and adjust treatment plans accordingly May perform Ultrasound Mist therapy 2-3x weekly (experience in Ultrasound not needed, will train) Administrative work from home duties Ratio of administrative and clinical duties will vary based on need Part time may expect some local travel up to 2-3 hours Full time may expect frequent travel mostly within CT for full time travelers, we will assist in obtaining other state licenses Active Nurse Practitioner license in state of employment Minimum of 1 year of experience in wound care Ability to work independently and as part of a team Exceptional critical thinking and decision-making skills Excellent verbal and written communication skills Strong time management and organizational abilities Comfortable with technology and electronic medical records. Travel throughout Connecticut would be required.
