Position Summary Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions. The salary range for the role is $96.4228 - $96.4228 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need.
