Analyzes and helps to resolve complex clinical case management problems, working in collaboration with other NFP managers, clinical specialists, and other VNS Health professional staff, as appropriate. Provides guidance to individuals and team to identify and resolve internal and external customer issues. In consultation with team members, identifies staffing and service needs of designated geographic areas and recommends changes as needed. Interprets and implements NFP/DOHMH and agency policies, state and federal regulations, and collective bargaining agreements. Collaborates with other NFP team members to develop and maintain relationships with community health and social service providers to ensure reciprocal referral relationships that support NFP enrollment requirements and access to resources that meet family needs. Performs all duties inherent in a managerial role.
