Key Responsibilities: - Assist clients with daily living activities, including bathing, dressing, grooming, and meal preparation. - Provide companionship and engage clients in meaningful activities. - Perform light housekeeping tasks to maintain a clean and safe environment. - Monitor and report any changes in clients' health status to the care coordinator. - Follow individualized care plans and provide care with compassion and respect. - Ensure clients' comfort and safety at all times. - Maintain accurate and timely documentation of care provided. Qualifications: - HHA certification in the state of New Jersey. - Previous experience in home care or a related field is preferred. - Strong interpersonal and communication skills. - Compassionate, reliable, and dedicated to providing excellent care. - Ability to work independently and as part of a team. - Valid driver's license and reliable transportation are preferred. Essential Duties: Establish a compassionate environment by providing support to Residents and their families. Maintain documentation of Resident care services by auditing Resident and department records. Create appropriate care plans for residents Handle all supervisory duties for assigned units, including overseeing LPNs and CNAs that report to you Help establish and implement employment policies and procedures Maintain the standards of care for the units Review Resident records & quality of care Monitor overall care & review individual Residents’ cases Address questions or complaints brought forward by Residents or their families Education, Experience and/or Training: Long-term Care experience preferred Valid RN license required 2-4 years management experience. knowledge of City, State and Federal regulations pertaining to licensed skilled nursing facilities.
