Job Description Distinguishing Characteristics All positions are in the non-competitive jurisdictional class. Nursing Assistant 1: entry level; performs basic patient care duties under direction of a nurse, physician, or other clinician; may assist Nursing Assistants 2, physicians, and nurses in the performance of care functions. Incumbents at this level receive in-service training in a limited number of care procedures, leading to eligibility to qualify for appointment to Nursing Assistant 2. Nursing Assistant 2: full performance level; using detailed guidelines, standards, and procedures, and under the supervision of clinicians, observes patients and performs prescribed simple treatment procedures; attends to personal hygiene needs of patients; and performs limited cleaning and housekeeping duties directly related to patient care. Related Classes Rehabilitation Hospital Nursing Assistants perform similar functions to those of Nursing Assistants but are focused on rehabilitation activities and are only classified at DOH’s Helen Hayes Hospital. Licensed Practical Nurses are classified in a variety of State health care settings and observe patients for symptoms and reactions to treatments; treat patients for physical maladies as directed and under close supervision; and attend to patients’ personal hygiene needs. Incumbents are licensed New York State Licensed Practical Nurses. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. This information includes items such as observable physical behavioral changes, fatigue levels, and general behaviors and responses to routine treatments, such as foot soaks. Qualifications Nursing Assistant 1 Non-Competitive: possession of or ability to acquire the necessary knowledge, skills and abilities during the probationary period. Nursing Assistant 2 Non-Competitive: one year of satisfactory full-time experience in the provision of simple treatment and other patient care services under professional supervision. Description Job Title - Nurse Practitioner-Pre Heart Transplant - Mount Sinai Hospital - Full Time - Day A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patients condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. PRINCIPLES DUTIES AND RESPONSIBILITIES CLINICAL PRACTICE Assesses the health status of patients/families by means of comprehensive health history and physical examination. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. Documents findings and plan of care in the patient's medical record. Consults with physician according to established standards, policies, procedures, and protocols. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. Initiates referrals to specialty services and fosters continuity of care. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. Maintains patient/employee confidentiality in the management of information. Develops the plan of care based on the diagnoses and individual patient needs: Records, reports, and interprets the clients responses to the plan of care. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. Demonstrates accountability for own nursing practice. Considers ethical issues of professional nursing practice and adheres to the code of ethics. Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. Serves as a resource person to nurses and other health team members. Participates in peer review. Incorporates research findings in clinical practice. Participates in approved nursing and medical research studies. Keeps knowledge current by self-directed learning, literature review and attendance at educational programs. Participates in defining, maintaining, and interpreting standards of nursing practice. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED Ability to perform history taking. Ability to perform physical assessment techniques. Demonstrate and complete documentation of competency in specialty specific procedures according to established standards as part of initial orientation and annual performance review with collaborating physician or appropriately credentialed designee. Ability to interpret and apply principles of nursing research utilization. Ability to manage and coordinate a patient caseload. Work within a Collaborative Practice Agreement or Collaborative Practice Relationship Attestation as a member of the health care team in area of practice Physical stamina and manual dexterity to perform a variety of tasks and skills in area of assignment. Excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership EDUCATION REQUIREMENTS-See Attached for Additional Detail Licensure: New York State License with current registration as a registered nurse. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. Experience : Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. The salary range for the role is $84.5895 - $84.5895 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need.
