Schedule · 5 days a week 37.5 - 40 hours per week Respiratory Nurse (RN) We are searching for a highly motivated, flexible, organized, skilled respiratory therapist who has experience in a rehab center and possesses great communication skills to put the “therapy” into respiratory therapy. As a Post Acute and Long Term Care respiratory therapist, your role would involve providing specialized care and treatment to patients with cardio-pulmonary, respiratory conditions neurological disorders and physical conditions that manifest into respiratory disorders over an extended period. Your primary responsibility would be to assess, provide diagnostic testing, develop treatment plans, and provide treatment to our Skilled Nursing Facility population. Job Description ESSENTIAL JOB FUNCTIONS: 1. Provides patient treatments which meet patient needs and uses current treatment knowledge in accordance with facilities clinical procedures. This includes but is not limited to: · Performing accurate, comprehensive patient evaluations. · Developing an effective plan of treatment and obtaining appropriate approvals from a referring physician. · Communicating with supervisor and interdisciplinary team members regarding patient progress, problems, and treatment plans. · Instructing patient’s families and/or nursing staff in specified follow through programs. · Interviewing patients and families regarding previous level of functioning, lifestyle and current and future expectations for recovery. · Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice. · Evaluating patients within the required timeframe as per facility policies. · Provides patient care as appropriate and necessary, following the position description for the applicable clinical discipline in which licensure or certification is held. 2. Writes accurate, complete, and clear documentation in accordance with the facility’s regulatory, licensing, payor, and accrediting requirements. This includes but is not limited to: · Recording Screens/Resident Needs Reviews, evaluations, daily treatment notes, discharge summaries and walking rounds in accordance with facility Policies and Procedures. · Recording treatment charges accurately and ensuring that documentation is consistent with billing data. · Writing specific, objective documentation that is outcome oriented and details the patient’s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals. 3 . Consistently demonstrates facility interdisciplinary approach to patient care. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Contributes to professional development of self. · Maintaining current professional status and licensure. · Adhering to professional standards of conduct and clinical code of ethics and practice. · Implementing in-services and training. · Maintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers. · Meeting continuing education requirements of regulatory agencies. 5. Aids in building and maintaining the facility’s reputation as the unparalleled respiratory therapy provider by contribution as a professional member of the staff. · This includes but is not limited to oversight of the following departmental operations: · Consistently demonstrating the facility’s core values by interacting with co-workers, patients and families, with all other persons involved with the patient’s care. · Consistently demonstrating professional standards as outlined in the facility’s Employee Handbook and as communicated by the facility’s management. · Contributing to a positive work team by sharing information, using problem solving methods and accepting new ideas, constructive criticism, or advice from others. · Maintaining close communications within the facility with the director of nursing, nurse managers, and admissions coordinator to assist in development of the respiratory therapy program. · Establishing and maintaining an effective rapport with the facility administrator, MDS, Rehab Director, DON, and medical director. o Demonstrating superior customer service through courtesy, consistent follow-up and positive communication. 6. Demonstrates safe working practices . This includes but is not limited to: Identifying and correcting safety hazards or notifying the facility or the admin so that safety hazards will be immediately remedied. Maintaining work areas in safe and orderly fashion. Maintaining equipment in safe working order by facilitation annual calibration. Using proper body mechanics during treatments of patients and during support activities. Using universal safety precautions. Wearing appropriate protective equipment. Mobility, Transfers and Higher-Assistance Care Some clients may have limited mobility, be at high risk for falls or require significant physical assistance. Depending on the care plan, caregiver experience and completed training, responsibilities may include: Assisting with walking and balance Supporting clients using walkers or other approved mobility devices Supporting wheelchair mobility Assisting with bed mobility Repositioning clients in a bed or chair Helping clients move between a bed, wheelchair, chair, commode or toilet Using an approved gait belt when directed Assisting with sit-to-stand and stand-to-sit transfers Completing approved one-person transfers Following all two-person transfer requirements Using a mechanical or Hoyer lift when required Checking equipment before use Following fall-prevention procedures Requesting assistance when care cannot be completed safely Caregivers must never attempt an unsafe transfer or use equipment they have not been trained and authorized to operate. Meal Preparation and Hydration Depending on the assignment, caregivers may be responsible for: Planning meals according to client preferences Following approved dietary instructions Preparing meals and snacks Assisting with feeding Encouraging regular fluid intake Observing and documenting food and fluid intake Assisting with grocery organization Following safe food-handling practices Washing dishes Cleaning meal-preparation areas Reporting concerns related to appetite, swallowing or hydration Difficulty swallowing, coughing during meals, choking, vomiting or significant refusal to eat or drink must be reported immediately. Light Housekeeping Caregivers may be responsible for maintaining a clean, safe and organized client environment. Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Organization: Radiation Oncology Job Summary: The Advanced Practice Provider (APP) plays a vital role in delivering specialized care and managing the health needs of patients in either inpatient, ambulatory, or a combination of both settings. With advanced training and expertise, the APP focuses on subspecialized care, including chronic illness and disease management, ensuring continuity of care throughout the patient’s healthcare journey, which may span multiple years. We will continue to evaluate and improve the therapeutic effects of radiation from five fronts: Basic science research to identify new biological modifiers and therapeutic targets.
