Job Description: Maya Home Care is seeking a dynamic and driven Home Care Director of Nursing to join our team. The Director of Nursing (DON) is responsible for the overall clinical management, supervision, and coordination of nursing services provided by the agency. The DON ensures that all patient care is delivered safely, effectively, and in compliance with New Jersey Department of Health regulations, federal requirements, professional standards of nursing practice, and agency policies and procedures. The DON provides leadership to clinical staff, promotes quality patient outcomes, and maintains the highest standards of clinical excellence. Clinical Leadership Direct and oversee all nursing services provided by the agency. Ensure delivery of high-quality, patient-centered care in accordance with physician orders and individualized plans of care. Develop, implement, and maintain clinical policies and procedures. Provide clinical leadership and guidance to registered nurses, licensed practical nurses, certified home health aides, and other clinical personnel. Promote evidence-based nursing practice and quality improvement initiatives. Regulatory Compliance Ensure compliance with all applicable New Jersey Department of Health regulations governing Licensed Home Health Care Services Firms. Maintain compliance with federal, state, and local laws and regulations. Ensure adherence to OSHA, HIPAA, infection prevention, and patient safety standards. Prepare for and participate in state surveys, accreditation reviews, and audits. Implement corrective action plans as needed following surveys or quality reviews. Clinical Oversight Review and approve patient assessments, plans of care, and clinical documentation as required. Monitor the appropriateness and quality of patient care services. Ensure timely physician orders, recertifications, and clinical documentation. Evaluate patient outcomes and implement interventions to improve quality of care. Participate in case conferences and interdisciplinary care planning. Staff Supervision Supervise and evaluate nursing personnel. Assist with recruitment, interviewing, hiring, orientation, and retention of clinical staff. Conduct performance evaluations and competency assessments. Ensure staff maintain required licensure, certifications, and continuing education. Provide ongoing clinical education and professional development opportunities. Quality Assurance and Performance Improvement Direct the agency's Quality Assurance and Performance Improvement (QAPI) program. Monitor clinical indicators, patient satisfaction, incident reports, infection control data, and quality metrics. Analyze trends and implement performance improvement initiatives.
