Description Title - Nurse Practitioner Manager, Palliative Care at Home - Mount Sinai Home Health - Full Time - Day Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians. The team works in collaboration with patients’ healthcare teams to provide high quality evidence based complex palliative care and chronic care management and is designed to improve symptoms and QOL, reduce caregiver burden, reduce unnecessary inpatient utilization, and facilitate hospice when appropriate. Nurse practitioners will be responsible for the symptom management of chronically/terminally ill patients within an interdisciplinary team environment. This role will provide disease process and trajectory education to the team, patients and families. The NP will assess, order and interpret clinical testing, diagnose and treat symptoms related to a variety of chronic disease states to focus on palliative symptoms and advanced illness education. Responsibilities • Responsible for clinical and operational oversight. • Develops strategies to drive service line growth, clinical excellence, and attract and retain staff in collaboration with leadership. • Collaborates with leadership to create financial strategies that align with the organization's goals and objectives. • Serves as an expert and resource in symptom management, demonstrating sound clinical judgment and prescribing proficiency as a nurse practitioner/clinical leader. • Facilitates family meetings and goals of care conversations. • Responsible for the onboarding, orientation, and ongoing clinical education of the team. • Promotes a team environment based on company mission, vision, and values. Qualifications • Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. • Licensure: New York State License with current registration as a registered nurse. • State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: • a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. • b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). • c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR • Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Optum Senior Community Care provides the Patient Connect Program for United Healthcare members in a short stay/transitional setting with focus on reduction in 30-day hospital readmissions, improved completeness and coding accuracy of diagnosis and medical record documentation, increased closure of STAR/HEDIS quality measures, improved completion rates of Advanced Care Planning Directives, and improved patient and family satisfaction and discharge and post discharge support. The Optum practitioner will provide a one-time comprehensive health assessment for the member in collaboration with the case management team to support better care coordination and health outcomes. This is an autonomous role that creates enormous satisfaction for the NP as you impact the care and support of our population. Required Qualifications: For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care For PAs: Current NCCPA certification and state licensure Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations Active and unrestricted driver's license In addition to a driver's license, access to reliable transportation Ability to gain a collaborative practice agreement, if applicable in your state Ability to navigate varied environments and to position oneself as needed to perform job duties Preferred Qualifications: 1+ years of clinical experience as an APC (long-term care setting preferred) Experience in geriatric medicine, long-term care, senior living or home care setting Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficiency with electronic medical records and technology Compensation for this specialty generally ranges from $90 - $155 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
