Consultant Public Health Nurse, 25 hours/Week (Part-time) The Bureau of Public Health Clinics has the mission of improving the sexual heath of all New Yorkers. To achieve this the Bureau provides direct clinic services to people seeking sexual health care, and services to sex partners; monitors disease trends; provides education and training to providers and community groups, conducts research and develop policies to improve sexual health and wellness. The Bureau of Public Health Clinics operates 8 STI clinics throughout New York City (NYC). The Bureau seeks to hire 1 part-time Consultant Public Health Nurse. This position will be working closely with the Regional Nursing Supervisor. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Provide a welcoming and efficient introduction to the STD clinic. - Adhere to the Nursing Code of Ethics, exhibiting high standards of nursing practice, promoting a safe and ethical work environment, practicing with compassion and respect for the inherent dignity, worth and unique attributes of every person. - Promote, advocate for, and protect the rights, health and safety of the patient. - Provide high-quality, patient-centered care in the BPHC clinics by providing patient support, assessment, medication administration and education on several diseases such as but no limited to gonorrhea, chlamydia, syphilis, HIV, Monkey Pox and TB. - Assist with patient care regarding medication abortion services. Including rotating between the clinic care and the Telemedicine hotline to answer questions about and to provide medical and gestational age triage for medication abortion. - Conduct brief but sensitive problem-focused medical interviews including sexual history and questions regarding sexual orientation. - Obtain demographic data on all patients. - Make determination as to patients care needs based on chief complaints, symptoms, medical history and/or test results. - Provide hard copies intake forms for review by clinicians, counselors and social workers. When necessary, accommodate partner-related concerns. - Perform case-finding activities; counsels, refers, and follows up on cases. Cleans and maintains therapeutic equipment (i.e. nebulizers, oxygen concentrators, IV poles and pumps, wheelchairs, gerichairs, siderail pads, linen carts, med carts, treatment carts, tub rooms, etc.). Dates and labels all personal hygiene items. Removes and cleans all therapeutic equipment and returns to its appropriate storage area if a resident is discharged or expires. 11. Changes bed linen and incontinent care items as needed. 12. Follows established systems: a) Releases restraints and repositions residents according to established Care Plan. b) Ensures that all residents are out of bed according to established Care Plan (if appropriate). If a resident does not get out of bed, notifies Charge Nurse. c) Turns, positions and toilets residents as per Care Plan. d) Ambulates residents according to the plan of care and facility policy. C. Role Responsibilities-Interpersonal Skills: 1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information. 2. Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner. 3. Addresses resident and family satisfaction issues immediately by reporting them to the Charge Nurse in a professional manner. 4. Is aware of Resident Abuse Reporting Law. 5. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and enthusiastic attitude. D. Role Responsibilities-Safety: 1. Demonstrates ability to locate emergency equipment (i.e. emergency oxygen tank). 2. Understands and can demonstrate knowledge of role in Fire and Disaster drills. 3. Resident Care Responsibilities-Resident Dignity: 1. Addresses residents in a respectful manner. 2. Discusses confidential resident information in appropriate areas only. 3. Knocks on door prior to entering resident rooms, and maintains resident privacy and dignity when providing personal care such as bathing, dressing and toileting. G. Communication with the patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities. Actively communicates in the case conferencing sessions to establish best practices. Submits requests for re-authorization of “more visits needed” to payers timely. Coordinates Community Services for the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process. Maintain the Daily 24-hour Report and submit to Supervisor as required. • Direct CNAs in the completion of their duties. • Assist CNAs to help meet the needs of the residents when available (i.e. toileting, call lights, etc.) • Maintain a cohesive understanding with families/caregivers of the resident. • Complete accident/incident reports as necessary and chart all reports involving residents. • Report all complaints and grievances made by the resident to the appropriate source. • Perform any other pertinent duties as assigned by my supervisor. Participates in all admission decisions and may visit prospective residents before admission and/or designate a representative to screen prospective residents prior to admission. 4. Participates in daily or weekly management team meetings to discuss resident status, census changes, personnel, or resident complaints or concerns. 5. Evaluates the work performance of all nursing personnel, assists in the determination of wage increases and implements discipline according to operational policies. 6. Ensures delivery of compassionate quality care and nursing supervision as evidenced by adequate services and staff coverage on unit, absence of odors, general cleanliness, prevention of pressure wounds and apparent maintenance of optimal resident functions. 7. Demonstrates knowledge of and application of Key Clinical Quality Indicators and proactively monitors and implements systems to achieve and/or surpass company thresholds. 8. Exercises overall supervision of resident assessments and care plans. 9. Oversees or is responsible for the CNA certification program. 10. Reviews 24-hour report from every unit daily to monitor and ensure timely, effective responses to significant changes in condition, transfers, discharges, use of restraints, unexplained injuries, falls, behavioral episodes and medication errors. 11. Submits an infection control report findings to the QA Committee. 21. Actively participates as a member of the QA Committee. 22. Coordinates and/or develops on-going QA activities for nursing services to monitor nursing compliance with standards and regulatory requirements through rounds, interviews and record reviews. Compiles summary of findings for the Quality Assurance committee. 23. Helps the Administrator prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc. Reviews and reinforces important standards previously cited. 24. Participates in the preparation of the Plan of Correction response to an inspection survey and implements any follow-up QA required for any nursing allegations. 25. Communicates directly with residents, families, medical staff, nursing staff, interdisciplinary team members and Department Heads to coordinate care and services, promote participation in care plans and maintain a high quality of care and life for residents. 26. Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from residents or families about nursing services. 27. Monitors facility incidents and complaints daily to identify those defined as unusual occurrences by State policy and promptly reports such occurrences to Administrator for appropriate action. 28. Monitors complaint reports daily for allegations of potential abuse or neglect, or the loss or misappropriation of resident property and participates in these investigations. 29. Promotes compliance with accident prevention procedures, safety rules and safe work practices to prevent employee injury and illness and control worker’s compensation costs. 30. Assures staff is trained in fire, disaster and other emergency procedures. Evaluates staff performance during drills. 31. Interacts courteously with residents, family members, employees, visitors, vendors, business associates and representatives of government agencies. 32.
