Job Description LPN - Med/Surg/Tele Part-Time Guthrie Corning Medical Center is hiring for a part-time LPN to work and care for our medical/surgical/telemetry patients on the 2C nursing unit. This position is scheduled to work 24.0 hours/week and 48.0 hours/pay period and is eligible for benefits. We utilize the team nursing model which allows you to work at the top scope of your nursing license. In addition to working with your primary registered nurse and patient care partner, your patients are supported by our multidisciplinary teams consisting of social work, care coordinators, physical therapy, occupational therapy, pharmacy and palliative care. The 2C nursing unit consists of 24 beds with central monitoring capabilities. Position Summary: Assist physician with duties necessary to support the needs of the practice working within and to the maximum scope of license Education, License & Cert: LPN ‐ license based on corresponding position location Experience: One‐year experience in healthcare field preferred but not required Essential Functions: 1. Directs patient to exam room and prepares patient for exam or treatment by identifying height, weight, respirations, blood pressure, temperature, additional testing, and records same on chart. ESSENTIAL DUTIES & RESPONSIBILTIES: By following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following: • Maintain a healthy, safe environment. Providing nutritional and environmental support. • Assist with personal hygiene, toileting, dressing, walking and feeding the client. • Position the client when instructed to do so. • Assist with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor. • Remind the client to take medications. • Assist with payment of bills ONLY when authorized to do so and it is documented on the assignment sheet. • Run errands when authorized to do so. Documentation on the Aide Charting Sheet daily. • Report any important information or changes in the client's condition to the agency and/or the nursing supervisor when necessary. • Use of medical supplies and equipment when appropriate. • Emotional support and the development of a trusting relationship between the aide and the client. • Encouragement of client independence. QUALIFICATIONS/EDUCATION: • Must be eighteen (18) years of age or older. • Must have a certificate from a Home Health Aide training program consistent with State requirements with evidence of ongoing in-service education to maintain the certificate. • Must have the maturity necessary for establishing and maintaining a good work relationship with the client, the client's family and agency personnel. • Have a sincere interest in and a sympathetic attitude towards caring for the sick and disabled. A HOME HEALTH AIDE MUST: • Get along well with others. • Be dependable and trustworthy. • Maintain compliance with all agency policies and procedures. • Maintain in-service requirements as specified in State and agency regulations. Enjoy working with others. • Be a good listener, exhibit empathy for the sick, disabled and elderly, and be sensitive to the feelings of others. • Be neatly dressed and have good personal hygiene habits. • Maintain good health habits with regular physical and dental exams. • Wear an identification badge while working. FUNCTIONAL ABILITIES: • Able to lift, turn and transfer clients • Able to lift up to 50 pounds. • Must be able to stoop and bend. • Visual/ hearing ability sufficient to comprehend written and verbal communication. • Able to verbalize so that the average person can understand. • Provide direct care according to the established client plan of care. • Carry out a wide variety of tasks under the direction of the nursing supervisor. WORK ENVIRONMENT: • Regular exposure to client elements and occasional stress. The hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
