Description Job Title: Clinical Nurse-GI-Med Surge-Stepdown-Mount Sinai Hospital-Per Diem/Nights The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Considers the patients values, preferences, cultural diversity, expressed needs and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Department/Unit: Float Pool Work Shift: Staffing Options Program (United States of America) Salary Range: $43,599.00 - $56,679.00 The Patient Care Associate (PCA): Activity Companion has the primary responsibility to assist the Registered Professional Nurse in consistently monitoring patients with a potential for self-inflicted or accidental injury, or with eating disorders to maintain safety. During the assigned work period, PCA: Activity Companion will collect patient data, complete personal care of the patient, perform point of care testing such as whole blood glucose, turn & position and/or mobilize patients, and assist with caring for the needs of assigned patients with a focus on progress toward discharge, including during transitions within the acute care stay. The PCA: Activity Companion remains within close proximity of the patient at risk for self-harm and is trained to provide support through provision of divisional activities, and maintenance of a clean and safe environment for healing. Essential Duties & Responsibilities include but not limited to: Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations. Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions. Assesses and evaluates patient needs for, and responses to, care rendered. Applies sound nursing judgment in patient care management decisions. Provides primary and emergency care for occupational and non-occupational injuries and illnesses. Administers over-the-counter and prescription medications as ordered. Collaborates with the nursing team to create a Plan of Care for all patients. Directs and guides ancillary personnel and maintain standards of professional nursing. Minimum Qualifications Graduate of a professional academic nursing program in which a Diploma, Associate Degree or Baccalaureate Degree is conferred. Must hold current NYS Registered Nurse license Obtains and maintains certification in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS) Minimum of 1-3 years of clinical RN experience is required Minimum of 1 year of Telemetry or ICU experience is required Ability to improve job performance through continuing education. Possesses knowledge of quality concepts, principles, and problem-solving tools and techniques and demonstrates ability to apply in support of departmental and patient care quality improvement. We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients' homes in accordance with an established care plan.
