Up to a $25,000 sign on bonus for qualified RNs! Full Time day shift Registered Nurse RN on Acute Care 1 at Lourdes Hospital. Acute Care 1 a 36-bed inpatient at Guthrie Lourdes. This unit provides specialized care for patients with cancer, including active treatment, symptom management, and palliative and hospice services. In addition, the unit cares for patients recovering from strokes, focusing on close neurological monitoring, early mobility, and interdisciplinary rehabilitation. The unit also serves patients with a variety of acute medical conditions such as respiratory illnesses, diabetes, wound care needs, and other acute care diagnoses. The unit utilizes a team nursing model and is known for strong collaboration between RN and LPN teams to deliver safe, patient-centered care across the continuum of illness. Position Summary: The Registered Nurse (RN) provides professional, comprehensive nursing care for patients in an acute care environment. Accountable for the delivery of coordinated, safe, compassionate, therapeutic, evidence‐based quality care to patients and families, based on individual physical, emotional, and spiritual needs, and appropriate care strategies throughout the lifespan. Practices in accordance with the Nurse Practice Act in the state of employment, the American Nurses Association (ANA) Code of Ethics for Nurses, and the ANA scope and standards of practice. Essential functions and specific skills for the job are found in the unit specific onboarding checklist and in standards that are applicable to the specialty. RN Case Manager

POSITION OVERVIEW: The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. The RN must possess a working knowledge of OPWDD rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding and working effectively with diverse populations. KEY ESSENTIAL FUNCTIONS: Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. Uses nursing process to make continuing assessments of the individuals with developmental disabilities total health needs. Maintain direct interaction with consumers on a regular basis, but in no case, at a frequency of less than once per week. Perform nursing assessments, by direct physical examination, of each consumer upon entry into the program quarterly (at a minimum) thereafter. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the consumer’s condition. Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate. Monitor health and behavioral status, labs and efficacy of prescribed treatments. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
