Patients that are admitted to the Pediatric Unit will include those aged two (2) days to eighteen (18) years. We care for patients with a wide range of complexities which include High Flow Nasal Cannula, any patient that needs higher acuity medical attention will be transferred to another facility. In the case of a medical/surgical patient surge, adult patients may be admitted to the Pediatric Unit that have medical and surgical diagnoses requiring a mild level of care, with the expectation of a brief and uncomplicated hospitalization. Scheduling Requirements (Tier 3): Work a minimum of seven shifts per six-week schedule. Two of these shifts must be fulfilled on the weekend. Work two holidays per calendar year. Per Diem RNs will be paid at 1.5 times the rate for all hours worked on these holidays . Based on your hired shift, work at least two ‘difficult to staff’ days per calendar year (Easter, Mother’s Day, Halloween, ‘Black Friday’, Good Friday, Christmas Eve, Valentine’s Day or New Year’s Eve) Follow departmental call-out policy Make-up absences within the same schedule period. Failure to comply with this on two occasions in a year will result in withdrawal of the employee from Per Diem status. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. TCU and the Emergency Department (ED) are collaborating using a new split flow model. The split flow model allows emergency services to continue delivering care to arriving patients while the newly admitted patient is handed off to the TCU RN. The TCU is imbedded in and adjacent to the ED. Scheduling Requirement: 7PM-7AM every Saturday and Sunday. Key Roles and Responsibilities: The TCU TEAM provides high quality, continuous nursing care to patients who have been admitted but remain in the ED while awaiting an inpatient bed.
