Job Description The Manhattan Division of The New Jewish Home is searching for a full time in person Admissions Evaluator to join their team. Onsite location is at 120 West 106th Street, NY, NY 10025. The Duties And Responsibility Of The Admissions Evaluator Are Reviews PRI’s and all supporting clinical documentation for appropriateness to the facility. Serves as a liaison to hospitals through the metropolitan area and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to the facility. Completes all required clinical pre-admission assessment forms on all potential evaluated, gathering additional medical and social information as need for a thorough assessment. Begins the application process via telephone by disseminating information about The New Jewish Home programs; may gather psychosocial and financial information to complete applications with applicants and/or applicants. Documents all applicant clinical and psycho-social assessments. Submits monthly statistics and reports as requested by the Director of Admissions. Contacts Managed Care Companies to obtain pre-authorization; reviews level requirements to obtain appropriate level prior to admissions. Conducts tours of the facility with potential patients, families and or representatives. Interacts with HMO Case Managers prior to admissions, to review skilled needs and maximization of level. Perform a complete newborn physical examination and assessment of gestational age, discriminating between normal and abnormal findings. Conduct parent interviews and take family history. Collaborate with neonatologists concerning pertinent historical events, physical findings, diagnostic and therapeutic measures initiated on all admissions. Order appropriate laboratory tests and procedures. Perform diagnostic and therapeutic procedures necessary and appropriate for the care of the patient (from ELBW to LOA babies). Review and provide general care items involved in neonatal care, such as temperature stabilization, hyperalimentation therapy, feeding management, etc. Obtain patient history and performs assessment via observation, interview, and examination of the NICU patient. Order, perform and interpret diagnostic studies. Provide direct treatment and management of health conditions. Document on delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. Perform procedures, to include intubation, umbilical lines, lumbar puncture, thoracentesis/chest tube placement, PICC line placement, Participate in and attend to high-risk deliveries including stabilization and management of complex critical needs of the newborn. Demonstrate strong newborn assessment, management, and delivery room skills. Participate in neonatal transport, if/when needed. Work collaboratively with experienced neonatologists and APPs in a team-centered atmosphere. Demonstrate excellent communication and interpersonal skills.
