In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families. Provide clinical recommendations to the IDG based on F2F assessment findings. Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings. Notify the hospice agency promptly if unable to complete F2F visits as scheduled. May provide F2F coverage for multiple provider numbers with approval. Clinical Nurse II- Duke Children's at Creekstone Location: 4709 Creekstone Dr, Durham, NC 27703 Job Summary: We are seeking a Clinical Nurse II in our Duke Children’s Specialty Creekstone clinic located in Durham, NC to be an integral member of the healthcare team and provide direct and indirect care to patients. Job Duties and Responsibilities: Provide nurse triage support for selected group of Children’s Specialty providers. Assessing patient needs via phone or inbasket when answering calls/messages. Assess medications and submit prescription refills to pharmacy based on standing orders, Initiating and following up on prior authorization submissions to insurance. Facilitating peer to peer reviews as needed. Documentation in electronic health record. Completing procedural tasks in clinic as needed including: collecting patient vital signs, medication review, administering vaccinations and insulin injections according to protocols and provider orders, ambulatory BP monitoring, specimen collection and correct labeling, assisting with clinic performed, provide patient/family education, bill for nursing procedures. Documentation of medications, vaccines, assessments in electronic health record. Monitor and initiate corrective action to maintain the environment of care, including equipment and material resources. Once oriented to the role: act as charge nurse on a shift by shift basis including providing unit leadership, ability to triage patient phone calls/EHR messages, interpret, respond, route, and document appropriately. Perform other related duties incidental to the work described herein. Job Eligibility Requirements: Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program. All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date. Twelve months of appropriate clinical experience is required. Twelve months of pediatric experience is required. Must have current or compact RN licensure in the state of North Carolina. BLS required.
