Summary The Heart and Vascular Screening Program Coordinator RN works closely with leaders from across the Rush System for Health to define and implement strategies and workflows to improve lung care for the Rush community. The Program Coordinator RN functions within the full scope of practice to identify and secure support and resources necessary for successful origination and implementation of heart and vascular screening initiatives. The continuum of care will extend throughout the Rush health system, primarily serving the outpatient community. The Program Coordinator is responsible for meeting program goals in a timely and cost-effective manner. Program success will be measured through performance goals, program metrics, and other monitoring systems. JOB DESCRIPTION Job Summary Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and “pop up” clinics. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Provides general medical care and care coordination to various and/or specific patient member populations – adult, women’s health, pediatric, and geriatric. • Performs comprehensive evaluations including history and physical exams for gaps in care and preventive assessments. • Addresses both chronic and acute primary care complaints, and demonstrates ability to ascertain medical urgency. • Establishes and documents reasonable medical diagnoses. • Seeks specialty consultation as appropriate. • Orders/performs pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptoms; works within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately. • Understands when a member's needs are beyond their scope of knowledge and when physician oversight is needed. • Creates and implements a medical plan of care. • Schedules appointments for visits when appropriate. • Provides post-discharge coordination to reduce hospital readmission rates and emergency room utilization. • Performs face-to-face in-person visits in a variety of settings including in-home, skilled nursing facilities, and public locations. • Performs face-to-face visits via alternative modalities based on business need, leadership direction and state regulations. • Orders bulk laboratory orders to target specific member populations. • Performs alternating on-call coverage to triage any urgent lab results and pharmacy inquiries and develops appropriate plans of care. • Participates in community-based “pop up clinics” to build relationships with communities, and address gaps in health care. • Drives up to 120 miles a day on a regular basis to a variety of locations within the assigned region. Drives beyond 120 miles as part of extended mileage may be required on special project days. Special projects may include an overnight hotel stay. • Obtains and maintains cross-state license in other states besides home state based on business need. • Collaborates with fellow nurse practitioners to develop best practices to perform work duties efficiently and effectively. • Actively participates in regional meetings. • May prescribe medications and perform procedures as appropriate. • Performs timely medical records documentation in electronic medical record (EMR) computer system. • On occasion, may be required to walk flights of stairs while carrying up to 50 lbs. of equipment. • Engages in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. • Local travel required (based upon state/contractual requirements). Summary The Ambulatory RN 2 identifies patterns of patient responses and can use past experiences to implement solutions for current situations. The Ambulatory RN 2 is a competent nurse who has mastered technical skills and utilizes evidence based interventions. The Ambulatory RN 2 continues to collaborate with other members of the health care team when the need for assistance is identified. Other Information Required Job Qualifications: Current license and credentials by the state they are assigned. At least one year of nursing experience required. Earned Nursing Degree at the Baccalaureate level or higher for those hired after January 1, 2014. All transfers to Rush University Medical Center or Rush University Medical Group from other Rush entities must have an earned nursing degree at the baccalaureate level or higher prior to the transfer. Chief Nursing Officer must provide approval for any RNs with ADN degree hired after 10/22/2020, based upon unit need. ADN RNs hired after 10/22/2020 must show continued enrolment towards a BSN degree with a signature of contract stipulating a BSN degree must be completed within 3 years of hire, Failure to do so will result in termination. ADN RNS are not eligible for leveling to RN3. Current CPR Certification. Analytical ability. Communication skills in order to lead and teach. The ability to check documents for errors, use a keyboard to enter or retrieve data, and closely examine specimens, images or reports. A specialty certification may be required for certain departments within 12 months of hire. Preferred Job Qualifications National Certification. Physical Demands Ability to walk throughout the Medical Center and to be standing or walking most of the designated shift. Ability to lifting or carrying objects 35-40lbs. and lifting and positioning patients, some of whom may exceed 300 lbs. Work conditions include performing procedures where carelessness could result in injury or illness and/or contact with potentially infectious materials and/or strong chemical agents. Utilizes evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communications according to state regulations, regulatory agency standards and organizational policies and procedures. Communicates current evidence-based strategies, standards of care and protocols with peers. Participates in and contributes to system and clinic based quality initiatives, research, development of care protocols and evidence based activities that improve patient care and outcomes. Demonstrates accountability for implementing policies related to safe and effective administration of medications, the use of medical devices in the delivery of patient care and in the performance of patient procedures. Applies the teaching- learning process to meet the learning and self-management needs of patients and families. Assesses patient safety needs and implements appropriate strategies to minimize risk during clinical visits, telephone encounters and electronic communications. Recognizes changes in patient status and consistently responds effectively to optimize patient outcomes. Recognizes patterns and problems in a specific patient population that require evaluation. Demonstrates the ability to prioritize care for complex patients based on knowledge acquired from previous clinical experiences. Utilizes competent assessment skills to holistically evaluate the physical and psychosocial needs of patients and families throughout the care continuum. Critically analyzes patient cases and effectively adapts the plan of care. Establish regular communication patterns that facilitate the transition of care for patients and families across settings.
