Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients. The Certified Nurse Midwife will provide inpatient care to patients at Marin General Hospital a varied number of days per week as needed, nights, weekends and some holiday shifts included. The Certified Nurse Midwife may also perform 1-3 clinical sessions in the outpatient setting (either at MHMN or the Marin Community Clinics) a week as needed.
